Objective: To evaluate the efficacy for the liposuction in the axillary lymph node dissection of breast cancer for preserve the axillary nerve,blood vessels and lymphatic,in order to reduce postoperative complications.Methods: 80 patients of operable and clinical stage I to IIIA with breast cancer in women randomized into observation group(40 cases)and control group(40 cases).Two groups of patients has no obvious difference(P?0.05),without history of radiation and chemotherapy,axillary radiotherapy preoperatively and scar disease,axillary lymph node without obvious fusion,limb edema,distant organ metastasis and severe organ function not congruent surgery taboo.All patients with postoperative follow-up of 3 months above.Observation group in the armpit area intraoperative injection of fat dissolving liquid,liposuction underwent lymphadenectomy after 10~15min,intact intercostal nerve,long thoracic nerve and the thoracodorsal nerve.Control group of 40 were received traditional axillary lymph node dissection.Two groups of operation by the same group of doctors.Groups in the total operation time,the average number of lymph nodes,postoperative drainage,subcutaneous fluid,limb pain,shoulder movement disorders,limb edema and other indicators were analyzed.According to BMI,the patients were divided into obese and non-obese group analysis,comparing the difference between the two groups of various indicators.Results: Observation group,total operation time(83.1±6.5)min,shortened than the control group total operation time(115.8±12.7)min(P < 0.05);the average 19 lymph nodes per patient in bservation group,much more than the control group of 13(P <0.01);postoperative drainage,significantly less than the control group(P <0.01),mean extubation time earlier than the control group;2 cases observed of limb pain,9 cases in the control group;shoulder movement disorders observation group 1 cases,8 cases in the control group;limb edema recent observation group did not occur,in the control group had six cases of upper limb edema,postoperative complications were significantly differences(P <0.05).Subgroup analysis showed that whether patients with obesity or not,the difference of each index of the observation group and control group have statistical significance,but in the number of lymph node cleaning,obese patients in observation group cleaning much more lymph nodes than the control group of obese patients(P < 0.01),and it is also difference in obesity patients of observation group(P < 0.05).Obese patients compared to non-obese patients in observation group and control group differences.Conclusions: The efficacy for liposuction in the axillary lymph node dissection of breast cancer has the following advantages:manipulated simpleness,Shorten the operation time,removes adequate lymph nodes,less nerve and vascular injury,and fewer postoperative complications,much safe and reliable.The expected results of this study was as mastoscopicin in the early period of the clinical application of subject,worthy of clinical further promotion.
