| Objective: To evaluate the clinical outcomes of Leuprolide Acetate combined with Mirena in adenomyosis patients with significantly enlatged uteruses.Methods: Forty patients with adenomyosis treated in our hospital from July 2014 to December 2016 were divided into combined group(n=20)and control group(n=20)according to the number of patients admitted to hospital by the random number table.The combined group was treated by leuprorelin combined with Mirena after laparoscopic resection while the control group was treated by the leuprorelin treatment.Before treatment(B),the first 28 days after injected of Gn RHa at the sixth cycles,before the LNG-IUS was placed,the first LNG-IUS after a month,3 months,6 months,9 months,12 months,18 months,24 months to observe uterine volume,CA125,menstrual volume PBAC score and dysmenorrhea VAS score.Results: The uterine size,CA125,menstrual volume PBAC score and dysmenorrhea VAS score of the two groups were significantly lower than those before treatment(P<0.05).The PBAC scores and VRS score in the combined group were respectively points 3 months,6 months,12 months after treatment,which were statistically lower than those of the control group(P<0.05).The uterine size and blood CA125 levels in the combined group were lower than those in the control group at 3 and 6 months after treatment,but there was no significant difference(P> 0.05).The uterine size and blood CA125 content at 9,12,18 and 24 months were significantly difference(P<0.05).The menopausal symptoms,irregular vaginal bleeding,ovarian cysts,breast pain and other adverse reactions of two groups after treatment were not significant difference(P>0.05).Conclusions: This study suggests that Gn RHa combined with LNG-IUS therapy can signifi cantly relieve dysmenorrhea,improve menorrhagia,have an important role in controlling inuterine volume,few side effects,especially for applications that require retention the uterus of adenomyosis patients. |
