Objective: This study aims to develop a set of touch exercises depend on Chinese older adults with cognitive impairment and sleep disturbance,and evaluate the efficacy of the touch exercise for senior citizens in improving their sleep disorders.Methods:1 The development of the touch exercise for senior citizens: It takes us ten months to develop the touch exercise for senior citizens.During this process,we referenced the Swedish Tactile care,the nursing situation of Chinese older adults with cognitive impairment and sleep disorders,abundant literature and we got the guidance of neurology experts,rehabilitation medicine experts,gerontologists,geriatrics department nurses.Our touch exercise for senior citizens contains two parts and fifteen sections.2 The implementation of the touch exercise for senior citizens: The study was conducted at a large nursing home in Shijiazhuang,Hebei.Participants were selected from a convenience sample of 38 elders with cognitive impairment and sleep disorders from February to September 2016,according to the inclusion/ exclusion criteria.On the basis of conventional care,the elder adults received touch exercise for senior citizens 3 times a week(every other day for 30 minutes)for 8 weeks.The touch exercise for senior citizens protocol was delivered by the trained nurse.The sleep condition were measured using wrist actigraphy,Pittsburgh sleep quality index,sleep diary,Epworth sleepiness scale at baseline,the fourth week,eighth week.Specifically,the study tested whether the massage contributed to improving the older adults' sleep quality and decreased anxiety and daytime dysfunction affect.All data were input by Epidata3.1 and analyzed by SPSS21.0 Statistical Software Package.Measurement data was expressed by x ±s,rang,percent.Repeated measurement of ANOVA was used to test the time effect of the massage.Matched t-test or ANOVA were used to test the salivary ?-amylase(sAA)changes before and after the touch exercise for senior citizens.Results: Of the 38 elders who participated our study,34 elders completed the touch exercise for senior citizens and data collection.1 After 8-week touch exercise for senior citizens,the average score of PSOI decreased from(14.15±2.96)to(6.24±1.52),total sleep time of ACT increased from(294.48±28.00)min to(351.63±28.17)min,sleep latency changed from(83.74±30.83)min to(27.70±10.22)min,sleep efficiency increased from(63.46±7.88)% to(84.18±3.96)%,total sleep time of SD increased from(265.03±28.34)min to(357.03±15.71)min,sleep latency changed from(100.22±33.36)min to(33.57±11.21)min,sleep efficiency increased from(55.58±6.76)% to(80.61±3.95)%,significant sleep improvement were observed(P<0.01).2 After 8-week touch exercise for senior citizens,the average score of ESS decreased from(6.74±3.47)to(5.26±1.75),the average score of PSOI-DD decreased from(2.26±0.84)to(0.82±0.63).Daytime dysfunction improvement were observed(P<0.05).3 At the fourth week,eighth week,sAA changed after touch exercise for senior citizens from(10.0,8.0)to(12.0,5.0),Anxiety was improved(P<0.01).Conclusion: The touch exercise for senior citizens is beneficial to improve the sleep quality of elder adults with cognitive impairment;both anxiety and daytime dysfunction belong to sleep disorder.It's worthy to be carried out in large-scale population in elder adults with cognitive impairment and sleep disturbance.
