The pathological mechanism of unruptured intracranial aneurysms(UIAs)is complicated, which is one of the most controversial topics in neurosurgery field. The treatment purpose of UIAs mainly is to prevent the rupture. The choice of treatment methods must weight the advantages and disadvantages between the risk of immediate rupture and future rupture. As a result, most of the patients with UIAs should be treated individually. With the develpment of the research,we have a better understanding of the pathological mechanism and rupture risk of UIAs.This article aims at discussing the natural evolution history of UIAs as well as the scientific evidence and the latest research progress to reduce the rupture risk of UIAs.In addition,we also make a summary in regard to the latest progress of drug treatment for UIAs.
