Objective: To investigate the dynamic change of blood pressure and its influencin g factors in patients with upper urinary calculi obstruction and hypertension after re moving obstruction,and to provide some theoretical and guidance basis for clinical treatment.Methods: From July 2014 to December 2016 in Affiliated Hospital of Zunyi Med ical College surgical treatment of 110 cases of patients with upper urinary calculi o bstruction and hypertension as the research object,The blood pressure of the subjec ts before and after 1 month was collected.Using single factor analysis method,co mpared sex and age of patients,calculi type,mode of operation,time of obstructio n,hydronephrosis,distribution of calculi,calculi size,grade of hypertension,renin(PRA)and angiotensin II(Ang II)value for difference of postoperative recovery of hypertension patients with upper urinary tract calculi.Screening univariate analysis w ith statistically significant variables,used Logistic regression to analyze the factors th at influence the recovery of postoperative blood pressure of hypertensive patients an d upper urinary calculi.The degree of postoperative systolic and diastolic blood pres sure(?2 test,P<0.05)were compared between different obstructive time,hydronephr osis,PRA and Ang II.In univariate statistical analysis,preoperative obstruction time,hydronephrosis degree,PRA and Ang II value with postoperative blood pressure dr ops statistically correlated(t test,P<0.05).Results: In the single factor analysis in statistics,there was a significant difference in the recovery rate of blood pressure between the 1 month after operation in pati ents with different calculi obstruction time,the degree of hydronephrosis,PRA and A ng?(P<0.05).there are no significant correlation between sex and age of patients,c alculi type,mode of operation,the size of calculi,calculi distribution,classification andoperation of hypertensive blood pressure drops(P>0.05).The rate of blood press ureand blood pressure recovery time of obstruction for more than 3 months after su rgery decreased lower degrees of obstruction time less than 3 months of the patients,the differences were statistically significant(P<0.05);moderate and severe hydron ephrosis in patients with postoperative blood pressure recovery rate and decrease of blood pressure were lower than the mild hydronephrosis patients,and the differenc es were statistically significant(P<0.05);PRA and Ang II value recovery rate and blood pressure of elevated blood pressure in patients with postoperative decline are lower than those of PRA and Ang II normal patients,and the differences were stati stically significant(P<0.05).Logistic regression analysis demonstrated that obstructio n time(OR=1.841),hydronephrosis(OR=2.997),PRA and Ang ?(OR=3.931)are t he risk factors for the continuation of hypertension after obstruction.Conclusion: some patients with upper urinary calculi obstruction and hypertension,the blood pressure can be restored after the obstruction is removed.These patients with obstruction over 3 months,moderate to severe hydronephrosis,elevated PRA an d Ang?have lower postoperative blood pressure recovery compared to these patient s with obstruction less than 3 months,mild hydronephrosis,PRA and Ang?related t o normal patients.Long term obstruction,severe hydronephrosis,PRA and Ang ?are the risk factors for the continuation of hypertension after obstruction.
