Objective: To compare placed drainage and do not placed drainage for patients in early pain degree,blood transfusion rate,hip motion,blood loss volume,complications and so on for the effect of clinical curative effect after total hip arthroplasty using a direct anterior approach.Methods: A prospective,randomised controlled study method,to compare the suction drainage or no-drainage for the nearly effect of assessment when using primary total hip arthroplasty by direct anterior approach.A total of 50 THR patients were randomised to autologous blood transfusion(ABT)(n = 25)or no-drainage(n = 25).Measured items included: Week of thigh root diameter before and after operation,hemoglobin,Hematocrit,coagulation function,D-2polymers,whether change dressings,volume of drainage,blood transfusion,pain degree,Blood loss,whether bruises,hematoma,whether deep vein thrombosis,postoperative hospitalization days,healing of incision,bed time,postoperative infection,hip motion and so on.Do statistical analysis comparison.Results: Direct anterior approach primary total hip arthroplasty in the early,and placed drainage of the hip joint activity(P< 0.05)and visual analogue scale(P < 0.05)were better than no drainage of place,but the postoperative drainage of hemoglobin level is greater than not drainage group(P=0.031),the drainage and not placed drainage transfusion rate comparison(16% : 8%),the total blood loss,postoperative hospital stay,postoperative week of thigh root diameter,apparent ecchymosis,change dressings,postoperative blood coagulation function,D-2polymers,take out stitches time,there was no statistical significance.Not found deep vein thrombosis of lower limbs and Pulmonary embolism and The hematoma formation in postoperative,two groups had no infection,placed drainage group and the difference is not placed drainage group were no significant(P > 0.05).Conclusion:Postoperative drainage area that can reduce patient pain and get better hip motion when primary total hip arthroplasty using a direct anterior approach.But the placed drainage of hemoglobin lost less and blood transfusion rate is more high.So weigh the pros and cons,Early after surgery in order to get good curative effect,it is recommended that the DAA-THA of postoperative routine should be placed drainage;The other perioperative rational application of hemostatic drug and prevention of thrombosis,can reduce the related complications after surgery.
