Clinical And Laboratory Correlation Study Of Rheumatoid Arthritis Associated Interstitial Lung Disease

Object To analysis the prevance and clinical data of rheumatoid arthritis with interstitial Lung disease(RA-ILD)patients,provide certain clinical basis for the early diagnosis and treatment.Methods Clinical data of 204 patients with RA in January 2014-July 2015 admitted to the xin jiang uygur autonomous region people's hospital retrospectively were collected,According to whether complicated with ILD cases can be divided into two groups,using the principle of statistical to analysis correlation of the two groups and RA-ILD group between Han and Uyghur,anti CCP antibody positive and anti CCP antibody negative,including its population information,clinical manifestation,laboratory and imaging examination.Results(1)Onset age,the averageage,duration of RA-ILD group significantly greater than patients without ILD[respectively(46.7 ± 15.9)years vs(40.8 ± 12.8)years;(56.1 ± 12)years vs(45.7 ± 13.7)years;(8.5 ± 8.4)year vs(5 ± 6.4)year,P<0.05].(2)Comparing RA-ILD group,the incidence of cough,sputum,chest tightness,shortness of breath,velcro rales is higher than patients without ILD(P<0.05).(3)RA-ILD patients of WBC,HGB,PLT,DAS28,ESR,CRP,C3,C4 and RF,GPI were no significant difference in pure RA(P>0.05).(4)Anti CCP antibody is significantly greater than patients without ILD [(1373 ± 1101.7)U/ml vs(1013 ± 98)U/ml,P<0.05].(5)Rate of positive of anti-AKA was statistically significant(?2=4.293,P<0.05),but there was no significant difference rate of positive of anti-nuclear antibody and anti SSA antibody(P>0.05).(6)Age,durations,WBC,PLT,HGB,DAS28,ESR,CRP,complement C3,complement,C4,RF,GPI,anti CCP antibody,AKA,ANA and anti-SSA antibody were no significant difference between han and urghurs nationality(P>0.05).(7)FEV,FEV1,FEV1% of anti CCP antibody positive and negative RA-ILD has no statistical significance in the pulmonary function(P>0.05).And DLCO between the two groups has significance difference [respectively(5.7 ± 5.7)L vs(7.8 ± 7.8)L,P<0.05).Conclusion Male,older,longer duration RA easily combine with ILD,and cough,sputum,chest tightness,shortness of breath,velcro rales happen more than pure RA.anti CCP antibody and AKA might be important antibodies associated with RA-ILD.anti CCP antibody of positive easily occur carbon monoxide diffusion damage.So improve the lung HRCT and function for those patients in time,to early diagnosis and treatment.
