Objectives To investigate the relationship between the main risk factors,coronary angiography and genetic factors and changes in blood lipid levels in patients with premature coronary artery disease(pCAD).Methods 1 A total of 805 CAD patients diagnosed by coronary angiography from January 2015 to January 2016 were retrospectively analyzed.Clinical data were collected and blood samples were collected and blood lipid and other biochemical indexes were measured.2 According to age will be divided into p CAD group(male?50 years old,female?60 years old)and late onset group(male>50 years old,female>60 years old),to explore the characteristics of the risk factors between the two groups,and the distribution of dyslipidemia between the two groups and between different types of CAD.3 To explore the characteristics of risk factors between different groups and different CAD types and the distribution of dyslipidemia rate.4 The Taqman-MGB probe was used to detect the LDLR gene mutation in suspicious FH patients with p CAD,and the influence of genetic factors on the blood lipid of p CAD patients was discussed.Results 1 In this study,a total of 805 cases of CAD patients,486 cases of pCAD,319 cases of late-onset group,Comparison of two groups of basic data and vascular lesions found,age(47 vs 55,P<0.0001),smoking(56.6% vs 49.1%,P<0.0001),family history(22.92% vs 15.97%,P=0.0173),BMI(26.52% vs 16.28%,P=0.0115),single lesion(36.12% vs 26.69%,P < 0.01),more lesions(63.88% vs 73.31%,P < 0.01).2Comparison of biochemical indicators between the two groups found that,TC(4.25 vs4.10,P=0.0236)mmol/L.3 Abnormal rate of dyslipidemia between the two groups was(79.41% vs 70.85%,P=0.0053),LDL-C abnormal rate(25.72% vs 19.12%,P=0.0298).4The abnormal rates of dyslipidemia were compared in three different types of patients with coronary heart disease,LDL-C(25.86% vs 22.30% vs 37.70%,P=0.0404),TG(49.14%vs 51.01% vs 67.21%,P=0.0478),HDL-C(25.86% vs 36.15% vs 22.95%,P=0.0359).5Logistic multivariate regression analysis was performed on statistically significant indicators of p CAD patients,TC(OR=0.559,0.329~0.950 95%CI),LDL-C(OR=1.991,1.080~3.668 95%CI),TG(OR=1.219,1.029~1.445 95%CI),Hcy(OR=1.022,1.005~1.039 95%CI),smoking(OR=0.289,0.202~0.411 95%CI).6 Logistic multivariate regression of the main risk factors of dyslipidemia in p CAD patients,male(OR=0.447,0.206~0.971 95%CI),coronary heart disease type(OR=0.598,0.385~0.929 95%CI),Glu(OR=0.776,0.651~0.927 95%CI),the difference was statistically significant(P<0.05).7A total of 89 cases of possible FH patients were found,including 3 cases of W462 X,3cases of A606 T and 1 case of D601 Y mutation.Conclusions 1 pCAD patients with younger age,and more easily associated with smoking,family history and obesity and other risk factors,p CAD patients with a single lesion-based,late CAD patients with more lesions based.2 The levels of TC,LDL-C and Glu in the p CAD group were generally higher than those in the late-onset group.3 p CAD patients with AMI type is the most common,but non STEMI patients most likely accompanied by dyslipidemia.4 High levels of TC,LDL-C,TG,Hcy and smoking are the main risk factors for p CAD.4 Smoking,dyslipidemia and high Hcy hyperlipidemia are the major risk factors for p CAD,male,coronary heart disease type maybe the key factors affecting dyslipidemia.gender,coronary heart disease type and blood glucose are key factors affecting dyslipidemia.5 The prevalence of FH in p CAD population is 5% to 10%.
