Clinical Observation On Decoction Of Tonifying Spleen Removing Phlegm And Smoothing Collaterals Against Early Diabetic Nephropathy

Objectives To observe the clinical effact of Decoction of Tonifying spleen,removing phlegm and smoothing collaterals(TSRPSC)on early diabetic nephropathy(DN).Methods Patients with early DN of phlegm and blood stasis syndrome were randomly divided into treatment group and control group,30 cases in each group.All patients were given Western medicine symptomatic treatment,including regulation on blood sugar,lipids,blood pressure and physical exercise,patients in treatment group were arranged to take TSRPSC Decoction,dosage being 500 ml,once a day.The clinical efficacy of the treatment group were compared with that of control group,including TCM symptoms,FBG,PBG,Hb A1 c,24 hours urine protein,BUN,Scr,blood lipids after 8 weeks of treatment,with which to evaluate its effectiveness in clinical application.SPSS19.0 was taken to analyze the results.Results 1 The clinical efficacy of early DN: the total effective rate of treatment group was 90%,and that of the control group was 66.7%.The effective rate of the treatment group was significantly higher than that of the control group,indicating that TSRPSC Decoction improving the clinical efficacy of early DN was stronger than that in the control(P<0.05).2 Two groups of TCM syndrome score: The score of TCM syndromes in both groups were lower than those before treatment(P<0.05)(7.83 ± 2.36)in the treatment group was significantly lower than that in the control group(9.57 ± 2.46),indicating that the treatment group could significantly improve the clinical symptoms(P=0.007<0.05).3Comparison of blood glucose and glycosylated hemoglobin in both groups: there was no significant difference between the two groups before treatment(P> 0.05).After treatment,FBG,PBG and Hb A1 c were decreased(P<0.01),indicating that both groups can reduce blood glucose.But after treatment compared with the control group,blood glucose in the treatment group was lower than that in control group(P<0.05).4 24 h Upro Quantitative comparison: there was no significant difference between the two groups before treatment(P> 0.05),The mean value of the treatment group(92.48 ± 17.88)was lower than that of the control group(101.90 ± 18.39),indicating that the treatment group was stronger(P<0.01),the difference was statistically significant decrease 24 hours urine protein quantification(P= 0.049<0.05).5 Scr,BUN comparison: there was no significant difference between the two groups before treatment(P> 0.05),indicating that the two groups were comparable,The levels of Scr and BUN in the two groups were lower than those in the control group(P<0.01).Compared with the control group,the treatment group was stronger than the control group(P<0.05),indicating that the treatment group could improve the renal function.6 Comparison of blood lipids: After treatment.Levels of serum lipids were decreased(P<0.01).Compared with the control group(P> 0.05),there was no significant difference between the treatment group and the control group,indicating that the control group and the treatment group were in terms of hypolipidemic effect.Conclusions TSRPSC Decoction can improve clinical symptoms,reduce early DN patients with blood glucose and urinary albumin,change renal hemodynamics and improve renal function to delay the progress of early diabetic nephropathy.
