?Objective?To explore the applications of the glycolipid level detection,plasma homocysteine detection,digital fundus photography technology,carotid ultrasound technology in clinical screening of cerebral small vessel disease,so as to provide basis for Community Clinical primary screening of cerebral small vessel disease.?Methods?1.A total of 36 cases(72 eyes)of patients who were diagnosed as CSVD from Apirl 2015 to Apirl 2016 were selected as the observation group,and the other 30 cases(60 eyes)of the healthy volunteers who had normal head MRI results in the same time were selected as control group.2.There were statistical comparisons of general clinical information,fasting blood glucose(FBG),glycosylated hemoglobin(HbA1c),total cholesterol(TC),total triglyceride(TG),low density lipoprotein cholesterol(LDL-ch),high density lipoprotein cholesterol(HDL-ch),plasma homocysteine(Hcy)between the two groups.3.Both groups underwent the digital fundus photography and Carotid artery ultrasonography.There were statistical comparisons of fundus changes and carotid plaque between the two groups.4.The degree of association between variables using Logistic regression analysis.?Results?1.Compared with the control group,in observation group,incidence of hypertension,and the levels of systolic blood pressure,FBG,HbA1 c,LDL-ch,Hcy were higher.And the levels of HDL-ch were lower(P<0.05).2.Compared with control group,in observation group,incidence of arteriovenous crossing sign,total incidence of fundus changes,and retinal arteriosclerosis grade 3,incidence of carotid ultrasound plaque,and carotid ultrasound plaque number were higher(P <0.05).3.In the observation group,the levels of Hcy,the incidence of carotid plaque and the average of carotid plaque of patients with fundus changes were higher than that of patients without fundus changes(P <0.05).4.Logistic multivariate regression analysis showed that,arteriovenous crossing sign was an important risk factor of cerebral small vessel disease.?Conclusion?At the grassroots community,advanced imaging techniques are difficult to popularize,so the use of simple and economical inspection method for Cerebral small vessel disease clinical primary screening is of great significance.Cerebral small vessel disease is related to the blood pressure,glucolipid metabolism,retinal vascular changes,carotid artery plaque.In this study,more than half of cerebral small vessel disease patients with arteriovenous crossing sign(AVN),and digital ocular fundus photography may be used as one of the CSVD Clinical primary screening methods.Digital fundus photography technology,cooperate to detecting Carotid artery ultrasonography,the levels of glycolipids and homocysteine,it may improve the clinical screening value for cerebral small vessel diease in grassroots community.
