Research On Effects Of Respiratory Mechanics Of ABCDE Bundle In Patients With Mechanical Ventilation

Objective: To monitor the levels of respiratory mechanics,blood gas values and prognostic of patients with mechanical ventilation during two different nursing strategies.To present a better nursing strategy which can improve and stabilize the levels of respiratory mechanics,oxygenation and prognosis of mechanical ventilation on patients who were provided clinical reference.Method: A total of 143 patients with mechanical ventilation were included in this survey.Two different nursing strategies were implied into each patient orderly:(1)usual group was conventional sedation and analgesia strategy.(2)bundle group was ABCDE Bundle.Compared the difference of Pplat,Ppeak,Cst,VT,PaO2,PaCO2,SaO2,PaO2/Fi O2,in two groups before and after the implementation of the two different nursing strategies,and the comparison of sedation and analgesic dosage,the incidence of 28 d mortality and delirium,mechanical ventilation time,ICU stay.The SPSS 17.0 statistical software was adapted in order to build the measurement data Meanwhile,the measurement data were presented as mean ± standard deviation,After the repeated measurement in different nursing strategies and groups,the difference was statistically significant P<0.05.Results: 1.Effect on respiratory mechanics: There was no difference before implementation(P>0.05),and the bundle group Ppeak,Pplat,Cdny and VT improved were significantly than the conventional group(P<0.05)after implementation.Compared with a group of different time monitoring,before intervention,bundle group 3,5,7day after Ppeak,Pplat,Cdny,VT changed was statistically significant(P<0.05);usual group,however,after 5,7days implementation,respiratory mechanics compared with before the intervention was statistically significant(P<0.05).2.Effect on blood gas: There was no difference before implementation(P>0.05).But after implementation,the levels of PaO2,Sa O2 and PaO2/FiO2 were higher than before(P<0.05).Bundle group PaO2,PaO2/FiO2,SaO2 with prolonged time gradually increased,better than the usual group(P<0.05);compared with a different set of monitoring points in time,usual group 5,7day and bundle group 3,5day of the intervention group 7day after each index of blood gas analysis before and after intervention,the difference was statistically significant(P<0.05);3.Effect of prognosis of patients with mechanical ventilation: The prognosis of Bundle group is better than the usual group(P<0.05),Bundle group can shorten the duration of mechanical ventilation and ICU stay,reducing the dosage of analgesic and sedative drug use,reduce the incidence of delirium and mortality rate of 28 days,which can significantly improve the prognosis.Conclusion: 1The two different nursing strategies could change the levels of respiratory mechanics and oxygenation of patients with mechanical ventilation.2.ABCDE Bundle may be more beneficial in terms of respiratory mechanics and PaO2/FiO2,To better improve the prognosis of patients and the management of sedation and analgesia in patients with mechanical ventilation.3.No severe complication occurred during the experiment,it is very clear that ABCDE Bundle is much safe and effective.
