Objective: To observe the effect of MMP-9 in elderly patients with rheumatic heart disease(RHD)atrial fibrillation(AF)expression of atrial fibrosis in patients,to explore the relationship between MMP-9 and atrial fibrillation in patients with senile RHD fibrosis.Methods: Screening of xi'an jiaotong university first affiliated hospital,xijing hospital,shaanxi province people's hospital cardiac surgery on May 28,2012 to August 13,2012 hospitalized from rheumatic heart valve disease in 32 patients with cardiac valve replacement surgery,according to the presence of atrial fibrillation were divided into two groups: group of16 cases of chronic atrial fibrillation.There were 16 cases of sinus cardiac rhythm.The basic clinical data of these patients were collected,such as age,sex,blood pressure,current history,electrocardiogram,and heart color.All build extracorporeal circulation in heart surgery in patients with cardiac arrest,before after extracorporeal circulation and right atrium before intubation open access right atrial tissue(0.3-0.5cm3),using HE staining and Masson staining to observe the degree of myocardial fibrosis in both groups,using real-time fluorescent quantitative of nucleotide polymer chain should reverse technology(RT-q PCR)detecting atrial tissue collagen type??? m RNA level,using Western blot test(Western blotting)atrial tissue for the expression of MMP-9.Results:(1)HE staining results showed that compared with patients with sinus rhythm group,the group of patients with atrial fibrillation atrial enlargement of muscle fibers,arranged loosely,clearly visible fibrous tissue hyperplasia.Masson staining results showed that compared with patients with sinus heart rate group,the group of patients with atrial fibrillation in the atrial tissue thickness uneven blue collagen fibrous tissue,significantly increase myocardial interstitial collagen deposition.The atrial fibrillation group was significantly higher in collagen than the sinus group,compared to the statistical differences(P<0.05).(2)In atrial fibrillation group of atrial muscle tissue samples I m RNA relative expression of collagen type was obviously higher than that of sinus heart rate group;Atrial muscle tissue samples of relative m RNA expression of collagen type III although the sinus rhythm group in atrial fibrillation group increased,but is no statistical difference between groups.(3)The atrial fibrillation group mmp-9 expression level was higher than the sinus cardiac rhythm group(P<0.05);The expression level of mmp-9 protein in atrial fibrillation group was positively correlated with type I collagen m RNA,left and right atrial diameter(r=0.511,0.534,0.736,P<0.05).Conclusion:The expression of MMP-9 in atrial tissue of patients with rheumatic heart disease and atrial fibrillation in the elderly may be one of the molecular mechanisms of fibrosis in patients with atrial fibrillation.
