Objective To evaluate the correlation between the mechanical characteristics of respiratory system and the degree of gastroesophageal reflux in patients with OSAHS,and to explore the possible mechanism of gastroesophageal reflux in patients with OSAHS.Methods A total of OSAHS patients were enrolled in the Department of Otolaryngology,Xinjiang Uygur Autonomous Region from February 2014 to February 2016.They were treated with routine pulmonary function tests, polysomnography(PSG),pulsed oscillatory system(IOS)There were 117 cases of OSAHS diagnosed and 95 cases of non-OSAHS patients.Among them,62 patients with OSAHS were accompanied by pathological gastroesophageal reflux(group A)and 55 patients with suspected OSAHS.Patients with OSAHS were not associated with pathological gastroesophageal reflux(group B);41 patients with non-OSAHS were accompanied by pathological gastroesophageal reflux(group C);54 patients with non-OSAHS were not associated with pathological gastroesophageal reflux Group D).The parameters of polysomnography, conventional pulmonary function parameters, pulse oscillation measurement parameters,24 h multi-channel impedance-p H monitoring parameters were compared and analyzed. Results There was significant difference in AHI and LSa02 between group A(OSAHS with GER),group B(OSAHS without GER),group C(non-OSAHS with GER),group D(non-OSAHS without GER)(P<0.05).There was no significant difference in FVC,FEV1,FEV1 / FVC,VC and TLC between the four groups(P>0.05),but there was no significant difference between the two groups(P<0.05).The scores of Rs and R5,R10,R15 and X5,X10,X15 and Fres were significantly higher in group A than those in group B,C and D(P<0.05).There was significant difference between group A and group B(P<0.05).The mean value of UES between group A and group B was significantly higher than that in group A and R5,R10,R15,(P>0.05).There was no significant difference between the total number of reflux and the number of acid reflux and De Meester scores(P>0.05).There was no significant difference between the resting pressure and the residual pressure, the mean resting pressure and the residual pressure and the weak acid reflux(P<0.05).There was significant difference between group A and group B,group C and group D(P<0.05).There were significant differences in the total number of reflux, acid reflux and De Meester scores(P<0.05);LSa02 was negatively correlated with R5 and R20(P<0.05).The number of Zrs5 and R5,R10,R15 and the total number of reflux in OSAHS patients were significantly correlated with R5 and R20 in OSAHS patients(P<0.05)De Meester score was positively correlated(P<0.05);X5 was negatively correlated with the total number of reflux and De Meester(P<0.05).Conclusion the mechanical characteristics of respiratory system in OSAHS patients were changed, the respiratory resistance increased and the elastic characteristics of the respiratory system, especially the decrease of lung compliance was related to the gastroesophageal reflux.
