The Application Of Omaha Classification Scheme In Nursing Assessment For Elderly With Type 2 Diabete Mellitus

ObjectTo develop a nursing problem assessment for elderly with type 2 diabetes in the framework of Omaha classification scheme through Delphi method,and provide a scientific and reliable assess tool for community nurses;to explore the effective of the assessment form in the community elderly with type 2 diabetes,find actual and potential health problems in the population.MethodThrough theoretical support,literature review,in the basis of Omaha classification scheme,combined with the characteristics of elderly patients with type 2 diabetes,Delphi method was used to modify and perfect a scientific and valid evaluation tool.To investigate the 180 cases of elderly patients with type 2 diabetes in Qingdao Songshan community hospital,and identify the health problems that clients exist by questionnaire survey.ResultsThe positive coefficient was 100%,experts authority level is 0.83,the important assignment mean >3.5,the variation coefficient was <0.25,two rounds of expert enquiry for Kendall concordance coefficient(w)were 0.383 and 0.415 respectively,carry on the test of significance are statistically significant.Finally,the assessment system of the elderly with type 2 diabetes was formed,consisted of 4 first level indicators,34 secondary indicators,and several tertiary indicators.180 cases of community elderly patients with type 2 diabetes have 2300 nursing problems,which occurs mainly in the field of health related behaviors(42.4%)and physical fields(40.2%).A high incidence of nursing problems occurs in blood sugar monitoring,drug therapy,foot care,nutrition,circulation,community resources,visual,etc.ConclusionsThe structure of the evaluation is clear,and the content is comprehensive,it is a scientific and reliable tool to assess the existing or potential nursing problems of the elderly patients for the community nurses comprehensively and effectively.The Omaha classification system helps the community nursing staff to assess the elderly patients with type 2 diabetes in the community comprehensively and effectively,form the concept of patient-centered,and provide theoretical basis for making aimed nursing plan.
