The Clinical Value Of Serum CA125 In The Diagnosis And Treatment Of Ovarian Endometriosis Cyst Rupture

Objective: To explore the correlation between serum CA125 and the rupture of ovarian endometriosis cyst and its clinical significance by testing the levels of CA125,then to provide theoretic basis for surgical risk prediction,preoperative assessment and postoperative follow-up and to direct clinical work.Methods: Selecting 150 patients treated surgically between January 2011 and December2016 from the department of gynecology,they were diagnosed as ovarian endometriosis cyst according to postoperative pathology.50 patients with the rupture of ovarian endometriosis cyst were defined as experimental group,while the other 100 patients without rupture was as control.Routine test,including serum tumor markers,such as CA199,CA125,AFP and CEA,blood,urinary and stool routine test,coagulation test,biochemistry,electromyography,chest X-ray,pelvic CT and type-B ultrasonic were performed.The surgeries were performed smoothly,and excised tissues were examined postoperatively by pathology.Full-automatic electrochemical luminescence analyzer was used to test serum CA125.The results were analyzed by statistical methods.Result:(1)The age of two groups were 33.1±7.8 and 34.76±7.3,retrospectively,with no significant difference(P>0.05);The menstrual cycle of two groups were 31.23 ±5.2days、33.41 ±5.4days,with no significant difference(P>0.05).The tumor diameter of the two groups mainly ranged from 5 to 10 cm.The comparison between them was X2=0.054、2.260、0.857)and P>0.05,indicating no significant difference.(2)The serum CA125 level of two groups was statistically significant(X2=28.125,P<0.05),but the CA199、CEA、AFP with no significant difference(X2=0.054、2.260、0.857,P>0.05)。(3)In experimental group,when the detection of serum CA125 was 35u/ml,the sensitivity of serum CA125 in predicting the rupture of ovarian endometriosis cyst was 90%,the specificity was 55%,the positive predictive value was 50%,and the negative predictive value was 91.67%.When the detection of serum CA125 was 70u/ml,the sensitivity of serum CA125 in predicting the rupture of ovarian endometriosis cyst was 82%,the specificity was 88%,the positive predictive value was 83.64%,and the negative predictive value was 92.63%.(4)Comparing the CA125 ≥ 70u/ml group and the CA125 ≤ 70u/ml group,the chance of ovarian endometriosis cyst rupture in the former increased hardly(X2=39.13,P<0.001)(5)Two groups of patients with postoperative 1 day,1 week follow-up serum CA125 level,the difference was statistically significant(X2=73.50,P<0.01),(X2=72.53,P<0.01),But 1month follow-up serum CA125 level,the difference was no significant difference(X2=1.53,P>0.05).Conclusion:There was no significant correlation between the age of patients and the rupture of ovarian endometriosis cyst,so as well the size of cyst.The higher the serum CA-125,the greater possibility of rupture.When CA125 increased as twice as normal,it is promised to become one of sensitive indicators of ovarian endometriosis cyst rupture.CA125 played an important role in the diagnosis and treatment of ovarian endometriosis and make sure the emergency surgery of cyst rupture preformed smoothly.
