Objective To investigate the positive rate of colorectal cancer specimens from patients with tumor adjacent site intestinal exfoliated tumor cells and its association with detection and tumor staging,pathological features,tumor size,tumor location,operation time,operation mode,sex and age of patients,in order to provide theoretical basis for the distal intestinal lavage and intestinal cavity chemotherapy tumor of proximal and distal intestinal cavity ligation,blocking the shedding of tumor cells spread.Methods 96 cases of patients with colorectal cancer were randomly selected in Shanghai Eighth People's Hospital,affiliated to Jiangsu University of gastrointestinal surgery cases,with complete data.3cm,8cm tumor of proximal and distal ligation was performed immediately for after tumor resection specimens.Connect pre-pumping full 30 ml saline syringe closed intestinal segments with side hole,with saline injection at the speed of 5ml/s and repeated suction and washing for 4-5 times,taking rinse and immediately centrifuged.With two cells precipitation,one is smeared,fixed and stained with HE,the remained portion was wrapped into soft clot cells adopting the plasma and thrombin.After fixation,dehydration,paraffin into cells paraffin sections and staining with HE,cell pathology detection under light microscope is performed,positive for tumor cells records.Record the results of two methods,the most records as the final result.Logistic regression model was established to analyze the influencing factors of the detection of colorectal cancer cell loss by single factor analysis and multivariate analysis.Results In this study,96 cases of colorectal cancer were included,of which 47 cases are positive and the positive rate was about 48.96%.Single factor analysis results showed that patients with tumor gross type,tumor size,differentiation degree,TNM staging are of statistically significance for colorectal cancer intestinal exfoliated tumor cell detection positive rate(P<0.05);while gender and age of patients,tumor location,surgical procedures,operative time off detection results of tumor cells in colorectal carcinoma the cavity was not statistically significant(P>0.05).Multivariate analysis showed that tumor size and tumor size were the main dangerous factors that influences positive rate of tumor cells.Among them,the positive rate of exfoliated cells(giant type,mushroom umbrella type,bulge type)was 20.8%,the positive rate of ulcer type exfoliative cells was 52.2%,and the positive rate of exfoliative cells was(P<0.05).With tumor diameter less than 3cm,the positive rate of exfoliated cells was 15%;with tumor diameter greater than or equal to 3cm less than 6cm,the positive rate of exfoliated cells was 56.7%,tumor diameter greater than or equal to the positive rate of 6cm cells was 60%.The differences among these three groups are of statistically significant meaning,P<0.05.Conclusion Near intestinal cavity happened to those with colorectal cancer.Total positive rate of intestinal lavage cytology was 48.96%,which is closely tumor pathological type,tumor size,degree of differentiation,TNM stages.Therefore,expanding resection,intestinal segment ligation,intestinal cavity flushing to effectively block or kill free tumor cells,reduce or prevent local recurrence are of certain clinical significance,for those patients with colorectal cancerwhose tumor cell shedding near intestinal cavity are possibly positive.
