Objective:evaluate the serum aminopeptidase N(APN/CD13)concentration in rheumatoid arthritis(RA)patients with bone damage so that monitor the risk of bone erosion in ealy RA patients.Methods 1st part: 78 cases of RA patients were inspected with ELISA,which including 25 cases patients without bone erosion and 54 cases with bone erosion patients according to the results of X-ray and ultrasonic examination.To compare the differece of serum APN in two groups of RA patients,and analyse correlation between the concentration of cytokines and APN.2nd part:Flow cytometer technology examine the rate of apoptosis of synovial fibrosis cell induced by TNF-??bestatin;TRAP staining analyse the function of TNF-??bestatin to prompt the mature of osteoclast cells.Results:1st part:(1)serum APN concentration of RA significantly higher than that of normal control group.(2)the serum APN concentration of RA patients with bone erosion much higher than without bone destruction of RA patients,and the tendency of APN concentration was accord with the classification of X-ray results.(3)the correlation between the activity index of RA patients including DAS28,joint pain,swelling of the joints,ESR,CRP and APN concentration had no statistical significance.(4)in serum concentration of APN as predictors of RA bone destruction is more reliable than RF,anti CCP antibody.(5)the serum APN concentration and TNF-? had positive correlation,but the corrlation between APN and IL-17 had no correlation.2nd part:(1)TNF-?didn't induce the expression of CD13?CD14 on synovial fibrosis cells.(2)the rate of apoptosis of the control group lower than the TNF-?group,the rate of apoptosis of the control group lower than the bestatin group.(3)the high concentration of TNF-? group has higher apoptosis rate than the low concentration of TNF-? group.(4)the rate of apoptosis in 40h?52h?64h in TNF-?group has the same tendency as the bestatin group.(5)bestatin increase the rate of apoptosis of high TNF-? group.(6)in TNF-? group,TRAP staining is positive,in bestatin group,TRAP staining is positive?Conclusion:(1)The serum APN concentration had correlation with cytokine,and was accord with the severe of bone erosion.APN maybe is a monitor to evaluate the risk of bone erosion inRA patients.(2)TNF-? have a different effect on synovial fibrosis cell in different time and concentration.(3)TNF-? can be induced by APN,however,TNF-? have no effect on APN expression.(4)in TNF-?group,TRAP staining is positive and was inhibited by bestatin.
