Development Of Schizophrenia Prodromal Computerized Adaptive Testing

important diseases.Schizophrenia is a chronic serious mental illness.It has the characteristics of early onset and morbidity Even if the patient received a reasonable treatment,condition is often accompanied by mild symptoms,and mild symptoms may be increased to become another disease.Even if the majority of patients with schizophrenia have been treated to improve their condition,their social function has been affected by light or heavy damage,the patient's life requires the care of relatives,which gives patients and their families a heavy financial burden.Therefore,schizophrenia not only to patients with great pain,but also to the patient's family a great burden.The impact of schizophrenia after the outbreak is irreversible,but the study found that more than 70% of schizophrenic patients before the onset of some time there have been some signs,this time is called the schizophrenia prodromal.The studies found that intervention in the prodromal period,can effectively prolong the onset time and improve the prognosis effect,so the screening of schizophrenia before the screening identification becomes very important.Screening and screening of the test scores a lot,but there are false positive rate,with long low efficiency,complex operation and other shortcomings.The preparation of the previous scale in accordance with the classic metrology for analysis,convenient false positive rate,a comprehensive range of many problems,time-consuming and inefficient.Now there is no mature schizophrenia prodromal screening tool in domestic.The assessment of schizophrenia prodromal in foreign countries relies on structured interviews such as CAARMS and SIPS,which can be used to assess the presence of symptoms and the severity of the symptoms,but these methods usually need to be evaluated by a professional and assessed up to 2-3 hours.On the basis of this text,also developed a number of streamlined scale,but these streamlined scale false positive rate is high,nor is it a very good assessment tool.In this study,based on the existing scale entries,we aim to set up an item Library of Schizophrenia prodromal.Then we analyze the items with Item Response Theory(IRT).Finally,combined with computer technology,we develop the computer adaptive test(CAT).We aim to provide individual tests for each subject,and improve efficiency without losing accurate.Objective:Refer to the domestic and foreign research about Schizophrenia and Schizophrenia prodromal stage,combined with China's current demand,the study compiles a Schizophrenia prodromal self-assessment screening test.The text which is based on the item response theory(IRT)as its theory foundation,depends on the computer adaptive test(CAT).The text is used to screen the crowd who are in the high risk of schizophrenia prodromal.Method:1.To select the appropriate term of schizophrenia prodromal,translate,integrate revise,and compile the item Library of the Schizophrenia prodromal self-assessment screening test.Based on classical test theory(CTT),the study use the SPSS software to analyze the reliability and validity of the item Library.2.Based on item response theory(IRT),the study estimates the parameters of Library items.3.Realize the CAT of the Library,and verify its feasibility.Result 1.The development of item LibraryAfter researching the common schizophrenia prodromal scales,the study selected 6 kinds of scale as a source of item.Then translated them twice.After translation,integration,revision,the study developed a 86-items schizophrenia prodromal item Library,with 5 pairs of consistency test.There are significant differences between 27% groups in the scores of the 86 questions.Reliability analysis,the Cronbach 's Alpha coefficient of the Library was 0.957(p<0.05),indicating that the internal consistency of the Library was good.And the test-retest correlation coefficient was 0.884(p<0.01)after 8 weeks.The validity analysis,content validity,structure validity and criterion related validity of three kinds of validity analysis.The content of this Library is compiled on the basis of the foreign mature scales and consulted with the experts.Exploratory factor analysis showed that the Library was unidimensional,and the confirmatory factor analysis showed that the model fitted well,which ensured the construct validity of the Library.Combined with the results of the relevant domestic and foreign research,the PD-16 scale was used as the standard.The criterion validity of the Library was tested,and the correlation coefficient was 0.830(p<0.05),which proved the validity of the Library.The validity of this Library was good.2.IRT analysis of item LibraryThe Library was tested by unidimensional test.The 3PL model was selected according to the characteristics of the items.The IRT parameters were estimated by BILOG-MG 3.0.3.CAT of item LibraryAfter analysis 7 items whose parameter is over reference,remove 4 inappropriate items.The Spearman coefficient between CAT score and test scores was 0.749,p <0.01,and the correlation was significant.CAT test use less 80% items,and 70% of time than traditional test.Conclusion:It is feasible to evaluate the computer adaptive test for the prophylaxis of schizophrenia.
