Objectives To investigate the risk factors of upper gastrointestinal bleeding and portal vein thrombosis in patients with liver cirrhosis.Analyze the prognostic factors of upper gastrointestinal bleeding and portal vein thrombosis in patients,and to prevention and cure for clinical.Methods In strict accordance with the case and control of inclusion and exclusion criteria,416 cases of liver cirrhosis were collected from January 2006 to January 2016 in the affiliated hospital of North China University of Science and Technology and Tangshan Infectious Disease Hospital.According to whether complicated with upper gastrointestinal bleeding and PVT formation;Divided into cirrhosis group 278 cases(no upper gastrointestinal bleeding and PVT formation),Bleeding and PVT group 138 cases(at the same time or concurrent with upper gastrointestinal bleeding and portal vein thrombosis).Statistics may cause cirrhosis complicated by upper gastrointestinal bleeding and portal vein thrombosis factors,Such as: etiology of cirrhosis,Blood type,past medical history and family history.Routine blood test:Red blood cell,Hemoglobin,White blood cell,platelet,Liver function: Alanine aminotransferase,Aspartate aminotransferase,Albumin,Total bilirubin;Coagulation function: Prothrombin time,Activated partial thromboplastin tim,Thrombin time,Fibrinogen;Width of portal vein,Splenic vein width,Spleen thickness,Liver function Child-Pugh classification.Various complications of liver cirrhosis:Esophageal and gastric varices,ascites,infection,cholelithiasis,hepatic encephalopathy and so on.All the collected data were analyzed by SPSS19.0.Count data used chi-square test,rank data tested rank sum test,measurement data used independent sample t test.Expressed as mean ± standard deviation,P <0.05 that the difference was statistically significant.The statistically significant indexes were analyzed by unconditional logistic regression analysis,To evaluate the correlation and the correlation between various factors in upper gastrointestinal bleeding and portal vein thrombosis in patients with liver cirrhosis.Screening out the risk factors of cirrhosis complicated with upper gastrointestinal bleeding and PVT formation.Results 1 Liver cirrhosis group and Bleeding and PVT group comparison in Sex(?2=1.703,P=0.192);Age(53.85±9.86 vs 53.19±9.00,t=1.86,P=0.136);Diabetes(?2=0.261,P=0.610);Hypertension(?2=0.351,P=0.553);Blood type(?2=6.129,P=0.106);Cholelithiasis(?2=2.719,P=0.099);TBIL(51.41±85.38 vs 59.00±78.02,t=1.185,P=0.237);TT(13.57±4.57 vs 12.90±3.72,t=-1.292,P=0.198);PT(15.49±3.93 vs16.08±3.30,t=-1.959,P=0.051);APTT(38.38±10.41 vs 37.52±7.82,t=-1.415,P=0.158);FIB(2.52±1.04 vs 2.82±1.33,t=-0.075,P=0.940).The difference is not statistically significant.2 Liver cirrhosis group and Bleeding and PVT group comparison in Causes of liver cirrhosis(?2=25.983,P=0.001);Family history of liver cirrhosis(?2=4.499,P=0.034);Liver function Child-Pugh classification(?2=56.688,P=0.000);Esophageal and gastric varices(?2=52.321,P=0.000);Ascites(?2=95.679,P=0.000);Infection(?2=84.134,P=0.000)Hepatic encephalopathy(?2=38.725,P=0.000);RBC(4.02±0.87 vs 2.89±0.84,t=12.780,P=0.000);Hb(132.66±83.10 vs 87.33±25.26,t=7.091,P=0.000);WBC(4.93±±2.66 vs6.17±4.70,t=-3.116,P=0.017);PLT(173.80±34.92 vs 94.40±38.48,t=4.832,P=0.000);ALT(96.44±136.75 vs 82.34±122.01,t=3.763,P=0.001);AST(100.26±131.80 vs160.01±395.43,t=3.247,P=0.000);ALB(35.71±7.37 vs 31.24±6.57,t=4.405,P=0.000);DPV(12.47±±1.50 vs 17.33±2.49,t=-2.064,P=0.040);DSV(8.09±1.80 vs 9.83±2.31,t=-2.664,P=0.008);Spleen thickness(36.91±8.72 vs 47.23±8.35,t=-4.500,P=0.000).The difference has statistical significance.3 Multivariate Logistic regression analysis showed that platelet,diameter of Splenic vein,diameter of Splenic vein,spleen thickness,esophageal and gastric varices,ascites,liver function Child C grade associated with Liver cirrhosis group and Bleeding and PVT group.Conclusions Platelet count decreased,diameter of portal vein widened,diameter of Splenic vein widened,thickness of spleen thickening,esophageal and gastric varices,ascites,liver function Child C grade are risk factor of upper gastrointestinal bleeding and portal vein thrombosis in patients with liver cirrhosis.
