Object: To evaluate the effect of Low-dose Naloxone combined with ropivacaine or sufentanil-ropivacaine mixture on brachial plexus block under the guidance of ultrasound.Methods: A total of 100 patients of our hospital undergoing upper limb surgery,will be randomly divided into four groups,25 patients in each group.20 ml of 0.375% mesylate ropivacaine(Group D);20ml of 0.375% mesylate ropivacaine+10?g sufentanil(Group S);20mlof 0.375% mesylate ropivacaine +100ng naloxone(Group N);20ml of 0.375% mesylate ropivacaine+10?g sufentanil +100ng naloxone(Group N+S).All patients were performed on interscalene brachial plexus block under B ultrasound.The sensory block,motor block and other adverse reactions were observed and recorded at 5min,15 min,30min,6h,12 h,18h,24 h.Results:The sensory and motor block time of group D were(435.5±77.9min)and(350.2±69.8min),thatof group S were(831.7±52.0min)and(675.8±48.1min),that of group N were(933.0±117.1min)and(499.0±40.5min),that of group N+S were(919.3±59.0min)and(534.8±56.6min).There was no significant difference in sex ratio,age,height,weight,BMI and ASA between the four groups.The sensory block time of group N and group N+S were significantly longer than the group D and group S(P<0.05).The sensory and motor block time of group D were obviously shorter than other groups(P<0.05).There were no significantly difference in the onset time of sensory and motor block in all groups.There was no significant difference in VAS pain score in all groups at 5 min and 24 h after injection.The VAS pain score of group D was significantly higher than other groups at 6h and 12 h after injection(P<0.05).The VAS pain score of group D and group S were significantly higher than those in group N and group N+S at 18 h after injection(P<0.05).Four groups of patients did not appear dizziness,nausea and vomiting,lethargy,skin itching,respiratory depression and other adverse reactions.Conclusion: Add a small dose of naloxone(100ng)on 0.375% ropivacaine for brachial plexus block anesthesia with or without sufentanil,can prolong the duration of sensory and motor block,reduce postoperative pain,help patients with postoperative early functional exercise,and do not increase the adverse reaction.It is safe,reliable,a kind of nerve block method,and worthy of clinical application.
