Pulmonary Artery Banding Apply In Congenital Heart Disease With Pulmonary Artery Hypertension

Objeetive :Pulmonary This study is trying to discuss the application of Pulmonary Artery Banding in congenital heart disease with pulmonary hypertension.compareing the result in different age and sum up the experience of using targeted drug before PAB.Methods: From 2010 to 2016,98 patients with congenital heart diseases underwent PAB in anzhen Hospital.There were 54 male patients and 45 female patients with their age from 1to 336 months(average35.40±59.95 months)and body weight from 3.5 to 56 Kg(average 10.47±8.82 Kg).All the patients were divided into three groups according to the age: ?6 months(n=35),7~36 months(n=39)and >36 months(n=24).Accroding to the malformation,there are two purpose of PAB among this patients: functional single ventricle repair(n=52)and later biventricular repair(n=52).The intraoperative and postoperative clinical parameters of all participants were observed and follow-up,the sPAP and sPO2 were record to evaluate the effect of PAB and other treatment.Also there are 13 older patients have using cardiotonic durg,diuretic or the target durg to inprove their condition for PAB and the result is satisfactory.Result:the result of PAB in all the patients are sitisfactory because of the significant drop of mPAP and SpO2,There are three individuals dead during the perioperative period and the in-hospital mortality was 2.04 %.One patient adjust the size of band after the first PAB.Two patients have the complication of hydropericardium and been cured after the medicine treatment.There are 19 patients accept the second stage surgery,Surgery of single ventricular include a TCPC,6 right Glenn shunt,5 right Bidirectional Glenn shunt and 7 bilateral Bidirectional Glenn shunt.Surgery of Double ventricular include 5 Double Outlet Right Ventricle correct and 5 TECD correct.Conclusion:Pulmonary artery banding can significantly restrict pulmonary blood.the surgery indication of age can up to 3 years old,the usage of targeted medicine can consolidate the treatment of PAB.It can also help patients with complex congenital heart diseasesto get two-stage surgical correction later.
