Purpose Breast cancer is the most common malignant tumor in women.Surgical resection is the main treatment method.Most postoperative patients have a favorable prognosis.But recurrence and metastasis are likely to occur in some of them.It's difficult for regular examination such as mammography and USG to distinguish between postoperative recurrence and scar formation.MRI has a good time and spatial resolution,the advantages in the soft tissue imaging that other methods don't have.Theoretically it can provide more evidence for diagnosis.Therefore,this study use pathological and clinical as a golden standard,to explore the value of MRI to distinguish recurrence from postoperative scar.We aim to explore the clinical application value of MRI in postoperative breast cancer,and to determine the best scan sequence of MRI to find breast cancer recurrence.Materials and Methods From 2010 to 2016,52 postoperative breast cancer patients who had MRI and USG reexamination at the same time in our hospital were chosen,excluding those without reliable follow-up clinical information.Retrospectively collect MRI and USG diagnosis conclusion,lesion morphology and surrounding violations and other useful information.The clinical data of these patients were collected.Results Among 24 cases considered as recurrence by MRI,23 cases was confirmed by surgical pathology,another case was confirmed by clinical follow-up as misdiagnosis,MRI diagnosis of scar or no recurrence in 28 cases,all of them was confirmed by clinical follow-up or surgery.The sensitivity and specificity of MRI were 100% and 96.5%,as for USG,the data comes as 78.3% and 82.8%.The MRI(kappa=0.961)results is more corresponding with pathology to USG(kappa=0.61).Different sequence in MRI shows different reliability in diagnosis,the enhanced focus on early DCE image is more corresponding to recurrence focus.Conclusion MRI shows a high degree of accuracy in postoperative examination of breast cancer.The DC E manifestation differences is the best to distinguish recurrence from scar formation.Combined with the results of our study and related research,the sensitivity and specificity of MRI in breast examinations are superior to that of ultrasound and mammography.MRI can provide valuable information for the identification of postoperative recurrence and scarring.Combined with domestic and international guidelines for breast cancer.We suggest that MRI be used for high risk postoperative patients as a review method.
