Objective this thesis discusses the benign prostatic hyperplasia and pathogenic factors of patients who get benign prostatic hyperplasia with ?B prostatitis,and makes an option for prostatic hyperplasia's impact and treatment protocols by researching the pathological features and clinical manifestations of the patients with benign prostatic hyperplasia with ?B prostatitis and the sufferers with simple benign prostatic hyperplasia.Methods 283 patients suffering from benign prostatic hyperplasia who accepted transurethral resection of the prostate(TUPKP)were enrolled,they were from inpatient units of Department of Urology,Affiliated Hospital of Yan'an University,dated from Jan 2013 to Dec 2015.They are evaluated after admission by expressed prostatic secretion(EPS),four cup urine culture,NIH-chronic prostatitis symptom index(NIH-CPSI),prostate specific antigen(PSA)?internation prostate symptom score(IPSS),quality of life score(QOL),maximum flow rat(Qmax),postvoid residual volume(PRV),acute urinary retention(AUR)and so on.According to diagnostic criteria for department of urology in the 2014 edition,we selected 66 cases who suffers from benign prostatic hyperplasia combined with ?b prostatitis.Meanwhile,30 patients who get simple benign prostatic hyperplasia are randomly chosen as a control group.Then the related indexes of two groups of patients with BPH were analyzed comparatively.The comparative study of prostate tissue's pathological typing and grading is carried out in the light of NICKEL prostatitis degree and diagnostic criteria for major inflammatory types after TUPKP surgery.According to the "NICKEL" of the degree of prostatitis inflammation and the main type of inflammation of the diagnostic criteria,patients with TUPKP postoperativeprostate histopathological classification and grading control study.Result Postoperative pathological examination results suggest that 66 patients with type ? B benign prostatitis hyperplasia patients with varying degrees of inflammation,mainly based on the infiltration of stromal and glandular inflammation.According to different pathological types,they are divided into three groups.The proportion of inflammation degree(G1 mild,G2 / G3 moderate to severe)for different group were statistically significant(P <0.05).The difference between glandular inflammation and glandular inflammation,matrix inflammation were statistically significant(P <0.05).There was no significant difference between glandular inflammation and matrix inflammation(P> 0.05).Combined with type ? B benign prostatitis hyperplasia of patients with inflammation of the organization showed: glandular area mainly mild inflammation infiltration,glandular weeks,the main part of the main medium and severe inflammatory infiltration.Most prostate gland takes on mild inflammation;There was no significant difference in glandular area inflammation between the two groups(P>0.05).However,the comparative difference between the two groups in the inflammation degree of the gland and stroma is statistically significant(p<0.05).The contrast of IPSS score,QOL,QMAX,PRV,and AUR between the two groups of patients has statistic difference(p<0.05).Conclusion 1.The pathological inflammation of patients who are subject to benign prostatic hyperplasia combined with ?B prostatitis is a little severe.Inflammatory cells are mainly distributed in the prostate gland and stroma.It's difficult to diagnose exactly.So doctors should be cautious when they are taking surgical operations.2.Prostate gland of patients with benign prostatic hyperplasia combined with ?B prostatitis is mainly mild inflammatory infiltration.The glandular and stromal inflammatory infiltration are mainly moderate or severe.3.The glands,its surrounding and stroma of the patients with pure benign prostatic hyperplasia are generally mild inflammatory infiltration.4.The case rate of benign prostatic hyperplasia combined with ?B prostatitis is relatively high.Obstruction may be one of the most important factors.5.The clinical symptom of BPH patients complicated with ?B Prostatitis is much severe.The same as age,prostate volume,serum PSA level,it is also one of the risk factors affecting the clinical progress of BPH.
