Objective:To analyze the effectiveness of intravenous(IV)and intra-articular(IA)tranexamic acid(TXA)in reducing blood loss in unilateral pr imary total knee arthroplasty(TKA).Methods:A total of 78 patients undergoing unilateral T KA for knee osteoarthritis were included in a retrospective study from March 2015 to December 2016.Patients were divided into three groups.The TXA IV and IA group received combined administration of TXA consisting of 1g administered intravenously preoperatively and 1g diluted in 50 mL of saline solution administered intra-articularly after closure of the capsule.The TXA IA group received 1g of TXA adm inistered diluted in 50 m L of saline solut ion adm inistered intra-articularly after closure of the capsule.And control group C,not given TXA treatment.The differences of intraoperative and postoperative bleeding volume,hemoglobin reduction and postoperative knee function were compared between the three groups.The difference between the three groups was statistically signif icant by SPSS21.0 software.Results:Compared with the control group,the postoperative drainage,hidden blood loss,total blood loss and hemoglobin reduction were significantly decreased(P <0.05).There was no signif icant difference in the amount of intraoperative blood loss between the three groups(P> 0.05)because the tourniquet was used throughout the operation.There was no significant difference in the amount of occult blood loss,total blood loss and 72 H hemoglobin reduction between group B and C(P> 0.05).All patients of the three groups had different degrees of blood transfusion,but the difference was not statistically significant(P> 0.05).All pat ients had no deep venous thrombosis,pulmonary embolism and TXA-related complications.Conclusions: For patients with unilateral primary total knee arthroplasty,the use of TXA can signif icantly reduce the amount of blood loss and hemoglobin loss in patients,and does not affect the functional recovery of the knee after surgery.No thromboembolic complications were observed.
