BACKGROUND Chronic spontaneous urticaria(CSU),formerly known as chronic urticaria(CU).It refers to the dilation of small blood vessel in dermis layer,increased permeability of the skin,resulting in skin edema,characterized by a duration of more than 6 weeks,and the urticaria or angio edema.Due to the long course of chronic spontaneous urticaria,the etiology is unknown,conventional antihistamine treatment is difficult to control patient's condition,Seriously affecting the quality of life,so the pathogenesis of chronic spontaneous urticaria and treatment has been the research hotspot in dermatology.In recent years,the imbalance of Th1 / Th2 cells and abnormal expression of chemokines have been considered as an important factor in the pathogenesis of chronic spontaneous urticaria,but its regulation mechanism has not been reported.Treg cells and their related cytokines play an important regulatory role in allergic diseases,but their role in chronic spontaneous urticaria is less.OBJECTIVE In this study,we measured the proportion of Treg cells and the level of Interleukin-17,Interleukin-27,Interleukin-35,transforming growth factor-?1 in peripheral blood of chronic spontaneous urticaria patients and healthy people,to investigate the regulatory role of Treg cells and their related cytokines in the pathogenesis of CSU.METHOD 1.37 cases of patients with chronic spontaneous urticaria(19 males and 18 females,age 40.41±15.33 years)and 40 cases of healthy control(21 males and 19 females,age 41.85±14.44 years)were selected from from January 2016 to June 2016 in the First Hospital of Lanzhou University.The percentage of Treg cells in peripheral blood was detected by flow cytometry,and the levels of IL-17 and IL-27 in serum were detected by enzyme-linked immunosorbent assay(ELISA).2.45 case of patients with chronic spontaneous urticaria(17 males and 28 females,18-73 years old,age 39.13±13.17 years)and 43 cases of healthy control group(12 males and 31 females,aged 18-60 years,age 29.44±9.89 years)were selected from January 2014 to December 2015.These cases were detected for serum levels of IL-35 and TGF-?1 by enzyme-linked immunosorbent assay(ELISA).RESULT 1.The percentage of Treg cells and the level of IL-17 and IL-27 The percentage of Treg cells and the level of serum IL-27 in CSU patients were lower than those in the control group(p<0.05).There was no significant difference in serum IL-17 level between the two groups.The percentage of Treg Treg cells was negatively correlated with serum IL-17(r =-0.359,p <0.05),And Treg cells had no significant correlation with the level of IL-27.There was no significant correlation between Treg cell ratio,IL-17 level,IL-27 level and UAS score.2.The levels of IL-35 and TGF-?1 The levels of IL-35 and TGF-?1 in serum of patients with CSU were higher than those of the control group(p<0.05).There was no significant correlation between IL-35 level and TGF-?1 level(p = 0.589).Spearman analysis showed there is no significant relationship between two cytokines and the length of the disease(p> 0.05).CONCLUSIONS The levels of IL-35 and TGF-?1 in peripheral blood of chronic spontaneous urticaria patients were higher than those of healthy controls,suggesting that Treg cells may participate in the chronic injury process of chronic spontaneous urticaria through IL-35 and TGF-?1.The expression of IL-27 in peripheral blood and the percentage of Treg cells were lower than those in healthy controls.The expression of IL-17 was negatively correlated with the percentage of Treg cells,suggesting that there may be an imbalance in Th17 / Treg with chronic spontaneous urticaria.And IL-27 may be associated with the pathogenesis of chronic spontaneous urticaria.
