Part ?.The research about the clinicopathological characteristics for type 2 diabetesmellitus with primary liver cancer after resection ObjectivePathology consultation is the most acceptable standard for the diagnosis of primary liver cancer.In this research,the clinical information from these patients who had liver cancer and resection were collected.These clinical information were sorted into two groups,one of the group belongs to liver cancer patients and without type 2 diabetes mellitus(shorted for group NT2DM),the other one belongs to these patients who suffered from both liver cancer and type 2 diabetes mellitus(shorted for group T2DM).Finally,compare the clinicopathological characteristics of those two group patients to provide some references for clinical diagnosis and treatment.MethodsBased on certain standard of acceptance and omission,the clinical information were collected from those patients who had hepatic neoplasm or liver cancer or hepatocellular carcinoma and had received treatment from hospital during January 2010 to November 2015.After sorting all of information into two groups,the compared parameters will be gender,age,education degree,systolic blood pressure,diastolic blood pressure,smoking or not,drinking or not,hypertension or not,BMI,Child-Pugh,liver laboratory tests,size of tumor,pathological TNM staging and so on.Results1.After processing and analyzing the raw information,this hospital had treated 6268 patients of primary liver cancer in six years.Among those patients,their average age is 54.5±12.2 years old.And there are 4891 specimens of male patients,accounting for 78%,1377 specimens of female patients,accounting for 22%.The male patients to female ratio is 3.55:1.The average age of female patients 56.5±12.9 years is elder than male patients 54.0±11.9 years(P<0.001).In the former,the clinical information are divided into two groups,group T2 DM and group NT2 DM.In the group T2 DM,there are 421 specimens patients totally(male:349 specimens,female:72 specimens).In the group NT2 DM,there are 5847 specimens patients totally(male:4542,female:1305).The male to female ratio in the group T2 DM is 4.84:1,which is higher than the group NT2 DM,that is 3.48:1(P=0.013).The average age in the group T2 DM is 61.9±11.4 years old,which is elder than group NT2 DM,that is 54.1±12.1 years(P<0.0001).2.Finally only 748 specimens of patients had been made a detailed analysis.Among these patients,the average age is 52.4±11.2 years,the youngest patient is 12 years old and the oldest patient is 82 years old.There are 602 specimens of male patients and 146 specimens of female patients.There are 75 specimens belonging to group T2 DM and 673 specimens belonging to group NT2 DM.The male to female ratio in the group T2 DM is 14:1,which is higher than the ratio in the NT2 DM,that is 3.8:1(P=0.003).Other compared parameters(average age,average systolic blood pressure,average diastolic blood pressure,hypertension and body miss index)in group T2 DM are all higher than that in group NT2 DM.After analysis and comparing pathological characteristics,in terms of histology,the ratio of biliary-cell type and mixed type in the group T2 DM is higher than that in group NT2 DM.There is significant difference in the histology classification.In terms of differentiated degree,the high differentiation in T2 DM is obvious higher than that in the group NT2DM(P<0.0001).The degree of liver cirrhosis,hepatitis B and AFP in the group NT2 DM is higher than that in group T2DM(P<0.05).There is no significant difference between the two groups in terms of their smoking habit,drinking habit,laboratory tests,education degree,Child-Pugh,size of tumor and TNM staging and so on(P>0.05).ConclusionsThe male patients of liver cancer are more than female.There is certain relationship between type 2 diabetes mellitus and liver cancer.The ratio of biliary-cell type and mixed type in the group T2 DM is higher than that in group NT2 DM.There is difference between the group T2 DM and the group NT2 DM in terms of clinical characteristics.Part?.The influence of diabetes on patients with liver cancer after resection: A meta-analysisObjective Meta-analysis about the surgical resection of primary liver cancer in patients with diabetes mellitus one year and five year survival prognosis through search published literature databases.Methods Such data based as Pubmed,Medline,CNKI,CMCI.According to inclusion and exclusion criteria,collected the prospective cohort study of surgical resection of primary liver cancer patients,Revman5.0 software was used for meta-analysis,compared one and five year survival prognosis with diabetes and without diabetes.Results There were 11 cohort studies eventually incorporated in the meta-analysis.These studies were from the Asian region(8 studies from China and 3 studies from Japan),including 3920 subjects(774 specimens patients with diabetes mellitus and 3146 specimens patients without diabetic mellitus).The results showed that one-year survival rate: OR=0.74,95%CI(0.55,1.00),P=0.05,there was no statistically significant;five-year survival rate: OR=0.53,95%CI(0.43,0.65),P<0.00001,there was statistically significant;five-year recurrence rate: OR=1.93,95%CI(1.45,2.56),P<0.00001,there was statistically significant.There is no publication bias by Stata12.0 software testing(P>0.05).Conclusion Diabetes reduced the survival rate and increased recurrence rate the in patients with liver cancer after resection,diabetes may be a risk factor for liver cancer prognosis after resection.
