Objective: To study the clinical effects of the different approaches that Patellar lateral approach(patella with knee joint capsule incision)and femoral anterolateral approach,respectively,combining with LISS plate for type C distal femoral fracture.Methods: 43 patients of type C distal femoral fractures were collected form affiliated hospital of Shandong University of Traditional Chinese Medicine,between March 2014 and June 2016,including 22 patients with patellar lateral approach,21 patients with anterolateral approach.During the study period,recorded the patient information by cases investigation system and obtained the status of the patients recover through telephone follow7y79o-up and periodic reexamination.Recorded blood loss and the time consumption during operation.Using HSS grading treated postoperation recuperation of knee function,ultimately,all data from the two approaches were analysed with the SPSS statistical software to review and compare the clinical effect of two kinds of operative approaches for the type C distal femoral fracture.Results: Two groups of patients in age,sex,trauma mechanism,fracture type,carried out a statistical analysis on the types of fracture,there was no statistically significant difference(P> 0.05).The differences were significant for operation time,hemorrhage volume,between the two groups(P<0.05).The good rate of knee function in patellar lateral approach was higher than anterolateral approach,but the difference did not achieve statistical significance(P> 0.05).The complication rates ofthe patellar lateral approach showed lower,but there was not statistically significant(P>0.05).Conclusion: Age,sex,trauma mechanism,fracture type had no effect on treatment of type C distal femoral fractures through the patellar lateral approach and the anterolateral approach.The operative time and intraoperative blood loss of patellar lateral approach reduced obviously than anterolateral approach.The complication rates of the patellar lateral approach was lower than the anterolateral approach,and From the complication rates and knee functional score,patellar lateral approach and anterolateral approach to the clinical effect of the treatment of type C distal femoral nalysis fractures has no obvious difference.
