Objectives:To investigate the characteristics of temporomandibular joint in patients with skeletal class ? malocclusion, study the changes of tooth and bone and other changes in temporomandibular joint after type ? functional orthopedic treatment, and investigate the clinical safety and therapeutic effect of vacuum lamination activator,and provide theoretical guidance for the clinical treatment.Methods:Part 1: Twenty patients with skeletal class ? malocclusion in mixed dentition were included to the research objects from 2014 to 2016 in Orthodontic Department of the Affiliated Hospital of Kunming Medical University, recorded as group T1, After treated with vacuum lamination activator for functional Orthopedic treatment was recorded as group T4.All patients underwent cone-beam computed tomography scanning before and after treatment,and measure the dental and bone index and statistical analysis.Part 2:All patients were chosen from 2014 to 2016 in Orthodontic Department of the Affiliated Hospital of Kunming Medical University.Including twenty patients with skeletal class ? malocclusion in mixed dentition recorded as group T0;twenty patients with skeletal class ? malocclusion in mixed dentition recorded as group T1(same as Part 1);twenty teenager patients with skeletal class ?malocclusion in permanent dentition recorded as group T2;twenty adults with skeletal class ? malocclusion recorded as group T3.All patients underwent cone-beam computed tomography scanning and the corresponding indexes of temporomandibular joint were measured, analyzed and compared.Part 3:Twenty patients with skeletal class ? malocclusion in mixed dentition were included to the research objects from 2014 to 2016 in Orthodontic Department of the Affiliated Hospital of Kunming Medical University, recorded as group T1, After treated with vacuum lamination activator for functional orthopedic treatment was recorded as group T4(same as Part 1).All patients underwent cone-beam computed tomography scanning before and after treatment,and the corresponding indexes of temporomandibular joint were measured, analyzed and descriptive research compared.Results: 1.After treatment, the facial shape of the patients improved, the anterior teeth coverage was significantly reduced, dental arch dressing is better,type II relationship improved significantly, the molar from the distance relationship into a neutral relationship,cephalometric analysis showed angle ANB significantly reduced,and Mandibular anterior teeth without significant compensation.2.(1)The results showed that both sides of TMJ in patients with skeletal class II malocclusion in mixed dentition were found the smaller in frontal joint space,superior joint space,condylar width,glenoid fossa depth, articular tubercle angle,condylar angle in coronal plane,condylar internal and external diameter.And condylar angle in sagittal plane,outside joint space,condylar angle in cross plane in group in patients with skeletal class II malocclusion in mixed dentition are bigger.(2)Compare the research results of skeletal class II malocclusion in different ages,with the growth of age,the width?height?anteroposterior diameter and internal and external diameter increase,the width and height of glenoid fossa increased. 3.(1)When orthopedic therapy finished the width of condylar and glenoid fossa,condylar angle in sagittal plane,inside and outside diameter of condylar all increased.(2)After treatment,whether condylar or joint fossa on the surface of the continuity of bone still fine, not causing destruction of the surface osteolysisdid,no imaging changes that could cause TMJ disorder were observed.Patients whose posterior cant of condylar flat before treatment improved after functional orthopedic treatment; but flat anterior cant of condylar have no significant change after treatment.Conclusions: 1.(1 )After type II functional orthopedic treatment with vacuum lamination activator,patients with mandibular advance, occlusion change significantly,overjet decreased, and facial shape improved;(2)After type II functional orthopedic treatment with vacuum lamination activator, angle ANB significantly reduced, the relationship between the jaws significantly improved , pour maxillary anterior teeth corrected, no significant compensatory increase;(3)The product of vacuum lamination is simple,make good effect, and which has its own unique role that can be applied to the class ? functional orthopedic treatment.2.(1)Both sides of TMJ in patients with skeletal class ? malocclusion in mixed dentition were found the smaller in frontal joint space,superior joint space,condylar width,glenoid fossa depth;articular tubercle angle,condylar angle in coronal plane,condylar internal and external diameter.And condylar angle in sagittal plane,outside joint space,condylar angle in cross plane in group in patients with skeletal class ? malocclusion in mixed dentition are bigger.(2)The characteristics of the temporomandibular joint in the patients with skeletal class ? malocclusion in mixed dentition were: the condylar width and the internal and external diameter of condylar were slightly amaller; the condylar is located in front of the glenoid fossa; the joint fossa and the articular nodules were flat;the condylar rotates inward. Which is compatible with its functional status, and it is also the result of compensation of sagittal uncoordinated the jaws.(3)With the increase of age, the structure of the temporomandibular joint from naive to mature, as follows:the condylar volume increases, the growth of mandibular fossa, deepening of glenoid fossa,the angle of the posterior tubercle of maxillary tubercle increases,the anterior joint space gradually increased,the joint gap tends to be coordinated.3.(1)After type? functional orthopedic treatment with vacuum lamination activator,the posterior space, condylar width, joint fossa width, sagittal condylar angle,condylar diameter were significantly increased.(2)The temporomandibular joint has undergone adaptive remodeling which is suitable for the effect of the class ? orthopedic force and the orthodontic process.(2)In the detection of CBCT level, no obvious pathological changes were found in the bilateral temporomandibular joint,the use of vacuum lamination activator of class ? functional orthopedic treatment is safe and reliable.
