Predictors Of Recurrence After Pancreaticoduodenectomy For Distal Cholangi-Ocarcinoma

Objective: To investigate the predictors for recurrence of Distal cholangiocarcinoma following pancreaticoduodenectomy.This topic will be more comprehensive in patie-nts with preoperative factors,operative factors,tumor pathological factors,post-operative factors four aspects to study risk factors for distal bile duct carcinoma postoperative recurrence underwent pancreaticoduodenectomy.Better evaluate the prognosis of distal bile duct carcinoma following pancreati- coduodenectomy, the better for patients to choose suitable treatment, to ensure the quality of surviva.Methods : We retrospective analysis the records of 73 patients who underwent pancre-aticoduodenectomy for distal cholangiocarcinoma between January 2009 to March 2014 in the in the second affiliated hospital of kunming medical university.we divided the patients into relapse group and no relapse group base on whether or not recurrent in two years.The two groups were set to research the factors may affect relapse.Independent t-test and X2-test were used for univariate analysis, and logistic regression was used for multivariate analysis to investigate the predictors for tumor recurrence following pancreaticoduodenectomy.Results:In 73 patients,2 patients died in perioperative period,mortality was 2.7%.There are 71 patients be put in our this research.Following up 24 months perioperative,21 patients were found tumor recurrence,50 patients had no evidence of tumor recurrence.The recurrence rate was 29.5%?Univariate analysis showed that preoperative albumin (P=0.036),intraoperative blood transfusion (P=0.032),pancreatic metastase (P=0.032),symph nodemetastasis status (P<0.01), metastatic lymph no-de(P<0.01),ratiograde of cell differentiation (P=0.417) and TNM staging (P<0.01)were significant prognostic factors of tumor recurrence following pancreaticod-uodenectomy.multivariate analysis showed that preoperative albumin (P=0.005) and ratiograde of cell differentiation (P=0.007) were independent prognostic factors of the distal bile duct carcinoma postoperative recurrence underwent pancreaticoduoden-ectomy.Conclusion:Preoperative albumin,intraoperative blood transfusion,pancreatic metas-tase,symph nodemetastasis status,metastatic lymph node,ratiograde of cell different-iation and TNM staging were significant prognostic factors of tumor recurrence following pancreaticod- uodenectomy.Preoperative albumin and ratiograde of cell differentiation were independent prognostic factors of the distal bile duct carcinoma postoperative recurrence underwent pancreaticoduoden- ectomy.
