Objective : To analyze the basic situation,disease characteristics and survival rates of the new 3859 lung cancer cases in Yunnan Tumor Hospital from 2012-2015.To understand the characteristics of lung cancer and to provide the theoretical basis for the prevention and treatment of lung cancer in Yunnan ProvinceMethods: To screening the first diagnosis of 3859 lung cancer cases in Yunnan Tumor Hospital from Januaryl, 2012 to July10,2015.First, do a descriptive annlysis of various, factors.Secndly, the area is divided into Xuanwei Fuyuan area(XF)and non-Xuanwei Fuyuan area(non-XF), and compare the factors of the two different regions.Finally, do the survival analysis of each factor.Results:1.Descriptive annlysisThe average age of the all was 57.32±10.86 years old;2690 cases (69.8%)were male and 1167 cases were female. There were 788 patients(20.4%)of XF. The number of the patients who’s focus on the right is 2195(57.3%), left is 1634(42.7%). Smoking patients accounted for 1888(48.9%). 285(7.4%)Patients find the pulmonary lesions by physical examination. Adenocarcinoma accounted for1737(45.0%), squamous carci-noma accounted for948(24.6%), small cell carcinoma accounted for439(11.4%). Clin-ical manifestation are mainly about cough(72.8%), expectoration(58.9%), blood-stained sputum(27.3%). The proportion of patients with stage IV was the highest(50.0%).2.Comparative analysisThe total ratio of male to female was 1.58: 1 in XF.The total ratio was 2.57: 1 in non-XF.Adenocarcinom in XF accounted for 64.2%, and accounted for 40.1% in non-XF. The female of the two region are mainly adenocarcinoma(non-XF female adeno-carcinoma accounted for 66.1%, XF female adenocarcinoma accounted for 81.09%). The male of XF are mainly adenocarcinoma(53.5%), non-XF are mainly squamous carcinoma(36.6%). The proportion of the physical examination patients in XF (16.8%)is higher than the proportion in non-XF(5.0%). The proportion of the stage Ⅰ and Ⅱwere 24.5%, 13.0% in XF, the proportion of stage Ⅰ,Ⅱwere 8.9% and 7.1%.3.Survival analysisAccording to the survival analysis, the surviral rates of 1 year, 2 years and 2.74 year were 59.5%, 38.5%, 19.0%. The most significant factors for predicting the prognosis of lung cancer were patients’ BMI(RR=0:807,P<0.001), region(RR=0.744, P<0.001),TNM stage (RR=1.643, P<0.001) , chemotherapy (RR=0.641, P<0.001),radio-therapy(RR=0.777, P< 0.001), pathological pattem(RR=1.098, P<0.001), physical examination(RR=0.588, P < 0.001), smoking(RR=1.374, P < 0.001), ECOG score standard(RR=1.144,P=0.004), age(RR=1.164, P=0.003), ethnicity(RR=1.194, P=0.049).Conclusions:1.The proportion of male lung cancer patients is higher than that in female in Yun-nan Tumor Hospital, the adenocarcinoma is the most high pathological type. The low proportion of patients who find the pulmonary lesions by physical examination. Some of the patients are from XF and the proportion is high. Clinical manifestation are mai-nly about cough, expectoration and blood-stained sputum. Middle -late patients are hi-gher than the others.2.The ratio of male to female in XF is lower than non-XF. There are so many adeno-carcinoma patients in XF and the proportion of adenocarcinoma patients are higher than non-XF. The survival rate and the the proportion of the early stage are higher than those in other places. These results suggested that focused effort in cancer scre-ening and increased lung cancer awareness in XF population led to earlier diagnosis and improved overall survival of lung cancer patients compared with the rest of Yunnan Province.3.Patients’ BME, region, clinical TNM classification and clinical stage, chemotherapy,radiotherapy, pathological pattern, physical examination, moking, ECOG , age and ethnicity have significant effect on the prognosis and survival rate.
