Objective:Schizophrenia is one of the most serious mental illness,attack or violent behaviors are a common clinical symptoms of schizophrenia.TH,EEG is the current research focus.To investigate the change of TSH,FT3,FT4,TT3,TT4 and EEG related waveforms(?wave,? wave,? wave,? wave)at the left forehead(Fp1),right forehead(Fp2),left middle temporal(T3),right middle tempora(T4),left central(C3),right central(C4),left occipital(O1)and right occipital(O2)in the first episode schizophrenic patients with attack or violent behaviors and its relationship with clinical symptoms,in order to provide an objective basis for the study of schizophrenic patients with attack or violent behaviors.Method:Collect information about the first episode schizophrenic patients from the hospital and forensic outpatient clinic in accordance with the DSM-5 diagnostic criteria.PANSS was administe to the schizophrenic patients.Then assess their aggressive behaviors with the MOAS,and sort them into two different groups according to the analysis result: 57 patients in the group of aggressive schizophrenia(referred to as the Attack Group)and 43 in the group of non-aggressive schizophrenia(referred to as the Non-Attack Group).Blood samples were collected for measurement of thyroid hormone and use the Japanese photoelectric1200 C EEG to test amplitude of?wave,? wave,? wave,? wave in two groups.Results1.Comparison of general data: There were no statistically significant differences in age,sex ratio,educational level and course of disease between the two groups.2.Comparison of TH between the Attack groups and the Non-Attack group: there was no significant difference between TSH,FT3,FT4,TT3 and TT4 in the two groups(p > 0.05).The TT3 in the attack group was significantly higher than that in the non-attack group[(1.97 ± 0.58 nmol /L vs(1.68 ± 0.39)nmol / L,p = 0.003],and the difference was statistically significant.3.The EEG parameters of the two groups were compared:?wave: There was no significant difference in the amplitude of ?wave between the Attack group and the Non-Attack group at Fp1,Fp2,T3,T4,C3,C4,O1,O2(p>0.05).? wave: The ? wave amplitude of the Attack group was significantly lower than that of the Non-attack group at Fp1[(4.08±3.23)u V vs(6.69±4.57)u V,p<0.05]?Fp2[(4.01±2.96)u V vs(6.01±3.18)u V,p<0.05]?T3[(2.71±1.64)u V vs(4.48±2.82)u V,p< 0.05]? T4[(2.88 ±2.12)u V vs(4.43 ±3.05)u V,p< 0.05]?C3[(4.58±4.67)u V vs(6.36±3.55)u V,P<0.05]?C4[(4.45±4.39)u V vs(6.09±3.44)u V,p<0.05]?O2[(5.80±5.06)u V vs(8.07±4.94)u V,p<0.05],and the difference was statistically significant.There was no significant difference in the amplitude of ? wave between the two groups at O1(p>0.05).? wave: The ? wave amplitude of the Attack group was significantly lower than that of Non-Attack group at Fp1[(4.70 ± 3.19)u V vs(9.91±9.40)u V,p<0.05]?Fp2[(5.34±3.92)u V vs(8.27±5.97)u V,p< 0.05]? T3[(2.37 ±1.37)u V vs(3.57 ±2.46)u V,p< 0.05]?T4[(3.23±2.03)u V vs(4.32±2.77)u V,p<0.05]?C3[(4.54±3.56)u V vs(6.51±4.10)u V,p<0.05]?C4[(5.23±4.13)u V vs(7.19±3.81)u V,p < 0.05] O1[(4.81 ± 5.95)u V vs(7.38 ± 5.20)u V,p < 0.05] ?O2[(4.77±3.86)u V vs(8.23±5.48)u V,p<0.05],and and the difference was statistically significant.? wave: The ? wave amplitude of the Attack group was significantly lower than that of Non-Attack group at Fp1[(6.07 ± 5.76)u V vs(10.72± 10.35)u V,p<0.05],and the statistical significance was obvious.There were no significant differences in the amplitude of ? wave between two groups at Fp2,T3,T4,C3,C4,O1 and O2(p > 0.05).4.Correlation analysis:4.1 Correlation of TH,EEG and 5 factors of MOAS: TT3 and the total score was significantly positive correlation(r =0.207,p<0.05),TSH,FT3,FT4,TT4 and speech attack,property attack,self attack,physical attack,there was no significant correlation(p>0.05).There are significant positive correlations between ? wave amplitude and property attack at Fp2(r =0.310,p<0.05),and the ? wave amplitude at Fp2 is positively correlated with property attack(r =0.562,p<0.05).There were no significant correlations between ? wave,? wave amplitude at Fp1,Fp2,T3,T4,C3,C4,O1,O2 and speech attack,property attack,self attack,physical attack(p>0.05).4.2 Correlation of TH,EEG and 5 factors of PANSS: TSH,FT3,FT4,TT3,TT4 and the positive symptoms score,the negative symptoms score,the general psychopathological score,the total score was not significantly correlated(p> 0.05).There was a positive correlation between ? wave amplitude and negative symptom scale(r = 0.310,p <0.05).There was a positive correlation between ? wave amplitude and negative symptom score at O1(r = 0.218,P <0.05).There was a positive correlation between? wave amplitude and negative symptom score at O2(r = 0.292,p <0.05).There were not significant correlations between ? wave,? wave amplitude and the positive symptom score,the negative symptom score,the general psychopathological score and the total score at Fp1,Fp2,T3,T4,C3,C4,O1,O2(p > 0.05).Conclusion : The change of TH,EEG in the first episode of schizophrenia patients with or violent behaviors:1.Attack group compared with Non-Attack group: TT3 concentration was significantly increased,suggesting that schizophrenia patients or the occurrence of violence and endocrine function changes related.2.Attack group compared with Non-Attack group: the amplitude of the amplitude,? wave amplitude and ? wave amplitude of the frontal lobe and temporal lobe were significantly decreased,suggesting that the attack or violent behavior of schizophrenic patients was related to the decrease of amplitude.3.Correlation analysis:3.1There is a significant correlation between TH,?wave ? wave of the right frontal lobe and schizophrenia or the severity of violence.3.2There was a significant correlation between the amplitude of ?wave and ? wave of occipital lobe and the severity of schizophrenia.In summary,the study of these characteristic indicators of schizophrenic patients with attack or violent behaviors provides an objective basis for studying the mechanism of schizophrenia attack or violent behaviors.
