Objective:To find out the distributed and epidemiological features of Alzheimer's disease in the investigation area,and establish a clinical process and method based on the community as a platform for local patient identification and diagnosis.Method:Multi-stage stratified cluster sampling method was used to collect 4000 people aged above 60 from Hongta and Midu.Using unified questionnaire,AD8,MMSE were used to screening AD for the elderly,and a simple physical examination was carried out for the respondents.For patients with clinical diagnosis of AD,CDR was used for dementia classification,HIS for dementia classification,NINCDS-ADRDA criteria for identification,and combined with the corresponding clinical examination.Data was entered into special software and statistically analyzed by SPSS 17.0.Measurement data was presented by mean and standard deviation.Enumeration data was presented by rate or proportion.Chi square test,Trend chi square test and Fisher exact test were used to compare the rates.Logistic regression was used to analyze the multi factors.The population attributable risk analysis was calculated by the corresponding formula.Results:(1)Basic information:This study investigated a total of 4062 cases of elderly people,a total of 3977 cases of valid questionnaires were collected,the effective rate was 97.91%.1765 cases were male,2212 cases were female.Urban and rural elderly was 1977 and 2000 cases respectively.The respondents aged from 60 to 93 years old,the average age was 68.47(±6.57).The average age of males was 68.62(± 6.59),and the average age of women was 68.35(±6.55).Marital status is given priority to married(77.95%).The degree of education is mainly primary school(88.81%),high school or technical secondary school accounted for 11.19%.Respondents most often associate with neighbor(82.42%)and exercise(87.73%),but not often participate in social activities(27.74%);Urban elderly(23.17%)and rural elderly(31.11%)prefer reading newspapers and playing cards.(2)The prevalence of AD increases:A total of 93 patients with Alzheimer's disease were diagnosed,with a total prevalence of 2.34%.The prevalence of Alzheimer's disease is different in different sexes and age groups,the difference was statistically significant.The AD prevalence of female patients was higher than that of male ones,the difference was statistically significant.Among male patients,The older of age,the higher AD rates,the difference was statistically significant.The prevalence of Alzheimer's disease varies with age and region,the difference was statistically significant.Urban prevalence rate is higher than that in rural areas,the difference was statistically significant.The older of age,the lower the degree of culture,the higher AD rates,the difference was statistically significant.(3)There are 9 factors have relevant with AD(P<0.05):gender,age,residence,education,neighborhood,whether participate in social activities,whether drink tea,whether read and whether play cards.Unconditional single factor Logistic Regression analysis variables are:age(OR=1.041,95%CI= 1.012?1.071),residence(OR=0.239,95%CI=0.145?0.394),neighborhood(OR=0.740,95%CI=0.574?0.953),whether read(OR=0.221,95%CI=0.089?0.549),whether play cards(OR=0.383,95%CI=0.201?0.731).The result revealed that the adjusted PAR for the independent risk factors were as follows:residence 0.5859,neighborhood 0.1112,reading 0.7368,playing card 0.5435,The total PAR was 0.9604.In addition to the above four risk factors,other factors,such as gender,education,and smoking,do not affect Alzheimer's disease in the population by more than 3.96%.Conclusion:The prevalence of AD increases have lower rate of 2.34%.Female,advanced age are risk factors for AD.Living in the rural areas,a high degree of education,neighborhood contacts,actively participate in social activities,drinking tea,reading books,newspapers,playing cards may be protection factor to AD.The total PAR of residence,neighborhood,reading and playing card was 0.9604.In addition to the above four risk factors,other factors,such as gender,education,and smoking,do not affect Alzheimer's disease in the population by more than 3.96%.
