BackgroundThe morbidity of cerebral infarction in China is increasing year by year,so people bring attention to it.Screening risk factors for cerebral infarction is the effective means of preventing cerebral infarction,which has become the key to clinical diagnosis and treatment.According to Chinese ischemic stroke subclassification(CISS),cerebral infarction is divided into five types,which is widely accepted in China.Large artery atherosclerosis(LAA)cerebral infarction which progressed rapidly is one of the most common subtype,and the consequences are serious and the prognosis is poor.There are many risk factors for the incidence of this type at home and abroad.Studies have shown that Willis circle variation is a risk factor,but lack of multivariate analysis including Willis circle variation.ObjectiveExplore risk factors of LAA cerebral infarction according to CISS,and study the correlation between Willis circle variation and LAA cerebral infarction,to analyze the effect of Willis circle variation on LAA cerebral infarction by multivariate logistic regression analysis,screen out the high risk factors of LAA cerebral infarction and provide the relevant factors for clinical physicians,to prevent LAA cerebral infarction in the future.MethodsRetrospective selection of patients from January 01,2015 to December 31,2016 in our hospital.They were divided into LAA group and control group according to the diagnostic criteria of acute ischemic stroke and the CISS classification.Firstly,using univariate analysis selected risk factors related to LAA,again corresponding risk factors were analyzed by multivariate logistic analysis.Results(1)Univariate analysis: Gender,high blood pressure grades,Willis circle types,anterior communicating artery,fasting plasma glucose,triglycerides,body mass index were compared with control group,the difference was statistically significant(P< 0.05),others had no obvious difference(P > 0.05).(2)Correlation analysis:Willis circle types was positively correlated with ACoA;Smoking and drinking were positively correlated with men;Carotid plaques were negatively correlated with HDL and positively correlated with age and hypertension;LDL was positively correlated with TC.(3)Dummy variable assignment: Willis circle types generated three dummy variable,selects the Willis type circle Ⅰ as a reference type,the results showed: compared with Ⅰtype of Willis circle,Ⅱ,Ⅳtypes of Willis circle β > 0,OR = 2.603,8.409.For high blood pressure level generated three dummy variable,selects the high blood pressure level 0 do a reference type,the results showed that compared with high blood pressure 0 grade,high blood pressure 2,3 grade β > 0,OR = 4.331,4.5.(4)Multivariate logistic regression analysis: There were significant differences in Wald test between Willis IV,Hypertension 2 and Hypertension 3(P < 0.05).Conclusion(1)The univariate analysis found in this study: Gender,high blood pressure grades,Willis circle types,anterior communicating artery,fasting plasma glucose,triglycerides,body mass index were associated with LAA cerebral infarction.(2)Dummy variable Logistic regression results show that compared with Ⅰtype of Willis circle,Ⅱ,types of Willis circle Ⅳ were risk factors for LAA cerebral infarction(OR = 2.603,8.409);compared with high blood pressure 0 grade,high blood pressure 2,3 grade were risk factors for LAA cerebral infarction(OR = 4.331,4.5).(3)The multivariate Logistic regression analysis: Willis circle Ⅳtype,high blood pressure 2 grade,high blood pressure 3 grade had a high correlation with LAA cerebral infarction.
