Background Patients with CP/CPPS ischaracterized by urinary symptoms and pain,frequently with changes in psychoemotional status.In fact,anxiety and depression arecommonly todetected in patients with CP/CPPS.Chronic pain and anxiety/depression exist in patients affected by CP/CPPS alone or in combination.The most frequently used treatments for CP/CPPS include single or sequential treatments or multimodal approaches withantibacterials,analgesics,anti-inflammatory agents,alpha-blockers,pelvic floor rehabilitation to treat dysfunctional voiding,but there have been few studies performed in which anxiolytic or antidepressant medications,or both,have been used to control psychologic problems.Objectives To evaluate the efficacy of diazepam and?-blocker combined with behavioral therapyin the treatment of patients with CP / CPPS.Methods A total of 322 patients in the Urology Surgery from March 2015 to September 2015 were randomly assigned to observation group and control group.Treatment in the observation group consisted of a simultaneous oral administration of diazepam and ?-blocker.Treatment in the control group consisted ofalpha-blockers and placebo.Both groups received behavioral therapy.NIH-CPSI,SAS and PHQ-9 scores were performed at 2 weeks,4 weeks and 6 weeks after treatments,and the data were analyzed and evaluated.Results 1.There was no significant difference in NIH-CPSI,SAS and PHQ-9 between the two groups before treatments(P<0.05).2.NIH-CPSI score was positively correlated with SAS score and PHQ-9 score.3.UPOINT Clinical Phenotypic Classification System.Patients with more positive factors and the higher CPSI scoreindicatethe more severe symptoms together with a positive correlation between the positive rate of UPOINT phenotype and the CPSI score.4.At the 6th week,we observeda significant decline in scores of NIH-CPSI,SAS and PHQ-9 both in the observation group and the control group,compared withbaseline before treatments.The scores in the observation group declined significantly more than the control group,and the difference was statistically significant(P<0.05).5.The effective rate was 88% in the observation group and 69% in the control group at the 6th week.There was significant difference between the two groups(P<0.05).Conclusion In this study,the UPOINT Clinical Phenotypic Classification System was used to conduct a general,systematic,and comprehensive assessment of the patients to further standardize the inclusion/exclusion criteria,followed by a validated parameter evaluation to assess psychological factors in CP/CPPS patients.And the NIH-CPCI score was positively related with anxiety and depression degree.In the study,a satisfactory effect was made in patients with diazepam combined with comprehensive treatment,affirming the important role of anxiolytic drugs in the treatment of CP/CPPS patients with psychological symptoms.It showed a great significance for the clinician.Psychologicalstate is closely related to the patient's habits,lifestyle and the environment.So we suggest that patients' healthy lifestyle and habits also played a role.Despite the limitations,the results of the study proved the good effect of combined application of diazepam in CP/CPPS patients with psychological symptoms.
