The Development Of Nursing Assessment System Software For Warning Traumatic Sepsis

PurposeThis topic through the Traumatic Sepsis Nursing Assessment System Software to filter out high-risk groups of traumatic sepsis,expect to guide nursing staff to give its advance prevention intervention,thereby reducing the incidence of sepsis after trauma.Methods1.Collected the data of 227 trauma and infection patients who from a third rate teaching hospital ICU in Chongqing,2012 to 2015.Using SPSS and Excel,by T test and x2 test,Logistic regression analysis,ROC curve analysis to know the risk factors of traumatic sepsis,and build Traumatic Sepsis Nursing Assessment System.2.Collected the data of 475 trauma patients who from a third rate teaching hospital in Chongqing,2012 to 2016,to verify the effectiveness of the traumatic sepsis risk assessment table.3.By expert consultation method to establish the Cluster nursing precautions of traumatic sepsis.4.the Python software editing tool was used to construct nursing traumatic sepsis early warning evaluation system software.Results1.The system have 7 assessment factor:Iduration of invasive mechanical ventilation,Bblood p H within 24 h,SOFA into the hospital,Staphylococcus infection,Wound infection,Dysfunction system number into the hospital,The average of pulse pressure difference within 24 h.The risk score formula: risk score of traumatic sepsis = SOFA + 0.5 *dysfunction system number + blood p H + 0.5* The average of pulse pressure difference + 1.5* Staphylococcus infection + 1.5* wound infection + 0.5 * duration of invasive mechanical ventilation.The system was used to predict sepsis,it's sensitivity is 0.77,specificity is 0.81,the index is 0.59,the critical value is 5.8.2.In 475 trauma patients,the infection rate is 64.21%(305 /475),Sepsis rate is 43.78%(208/475),Using the system to predict traumatic sepsis,the high risk rate is 3.37%(206/475).the sensitivity is 75.48%,,specificity is 81.65%,positive predictive value is 76.21%,negative predictive value is 81.04%,leakage rate is 10.74%(51/475),wrong prediction rate is10.32%(49/475).3.After two rounds of expert consultation,establish the cluster preventive nursing measures what based on risk factors of traumatic sepsis,including early assessment;Close monitoring of vital signs,PH,etc;Nursing of bacterial culture;Nursing of wound infection;preventive infections of Mechanical ventilation.4.Python to be used to build nursing assessment what named is traumatic sepsis risk assessment system.ConclusionsTrauma is the cause of sepsis.Infection is the premise of sepsis.This study from the perspective of infection for the first time to analysis risk factor for traumatic sepsis,and build traumatic sepsis warning formula,and collected trauma patients information to validate its effectiveness.Then build a system what is convenient and concise,prediction effect is good,and suitable for nursing staff to use,Provide the basis for nursing assessment,expect to reduce the incidence rate of trauma patients with sepsis.
