The Study On Building Standardized Treatment Process Of Hand Burn Scar

Hand burn scar has been a difficult problem in clinical treatment.Surgery is a principal method for reparation of wounds and recovery of hand function and appearance,and postoperative rehabilitation is important to consolidate therapy.Currently most general hospitals at home and abroad have built their own standards and methods on the basis of long-term practice and exploration.However,they haven't built a unified,scientific,and standardized process.Meanwhile,hospitals in remote areas of China are limited to medical equipment and overall clinical technical level,in whichmost of clinicians haven't gotten well-trained and effectively treated patient with hand burn scar who have delayed the best treatment time.Moreover,some patients lack of science education and coordination consciousness,and they do not know how to do rehabilitation training,resulting even worse prognosis.Therefore,it is necessary for large domestic comprehensive hospital and district,county,small and medium-sized hospitals to build a scientific system of standardized treatment process.It not only has positive practical significance for the realization of hospital medical management and improvement of the treatment of hand scar,but also provides decision-making basis for the relevant medical management departments to implement the reform.The present study adopted the theoretical and theoretical research,the status quo and case study,the ideas of the process of building research,used literature research,case analysis,expert interviews,expert demonstration,etc.,and fully used successful experience and practice of treating the burn scar treatment and rehabilitation training in the hands of the high level comprehensive hospitals for reference to summarize and refine effective concrete measures and proposed a scientific system of standardized treatment process.The study is divided into four parts.The first part is introduction,mainly introducing research background,the aim,the significance,the research content,the research present situation,the basic concept definition and the research technical methods.A literature review summed up research progress of the current domestic hand burn after treatment and rehabilitation,and put forward the importance and necessity of the establishment of standardized treatment process;it defined the basic concepts,such as scar and process,clarified the construction principles of the treatment process,analyzed and grasped the key contents of the research,and designed the research ideas and technical routes.The second part is the construction of draft process.We searched literature,study the famous plastic monograph,and sum up the focus of the treatment of hand scar.Data of 56 cases of hand burn scar during December,2011 to December,2016 was collected in department of plastic surgery.After summing up the successful experience,the first draft of standardized treatment process of hand burn scar was put forward.The third part is the study on process construction.On the basis of draft process mentioned in the second part,interviews with 7 experts were conducted to supplement,modify and optimize the draft process.20 clinical experts were invited to demonstrate and validate the final treatment of the procedures,frameworks and contents of standardized treatment process of hand burn scar,forming the final result of this study.The forth part is results and discussion.The research results are summarized and four flow charts,including preoperative preparation,surgical treatment and rehabilitation training,and their concrete operation are constructed: we summarized three basic elements,including the correct surgical procedures covered by this process,the survival rate of graft skin,postoperative rehabilitation and other anti-scar treatment.In addition,5 basic characteristics of the correction process were summarized,including(1)achievement the system concept of preoperative,intraoperative,postoperative integration;(2)classification construction combined with clinical practice;(3)the highlighted construction concept of "patient as the core";(4)the scientific and rational operation standards reflected by the overall design;(5)The utility of application and reference for the treatment of other parts.At the same time,the clinical significance and application prospect of the study were prospected.The last is the summary.The research process is summarized.The innovation of the research are mainly embodied in three aspects:(1)the application of management theory and method combined with discussion of clinical practice;(2)the improvement of shortcomings in the current clinical medical treatment in our country;(3)the wide applicability of the conclusions of this study for general hospitals and primary hospitals.The inadequacies of the study and the ideas for further researches were proposed.
