Psoriasis is a common skin disease,the global incidence is about 1-3%,in addition to the skin performance affected psoriatic patients with physical,social and psychological,the other systemic diseases are also happening.More and more epidemiological studies have confirmed that cardiovascular risk factors of psoriatic patients are more common than the general population.Psoriatic patients with increasing mortality,mainly due to cardiovascular disease.At present,there is no studies on the Framingham score of psoriatic patients in China.We conducted a prospective case-control study to determine the risk factors for cardiovascular disease in psoriatic patients.From October 2015 to October 2016,66 patients with psoriasis vulgaris were collected,and 109 healthy individuals were used as control groups.All patients with psoriasis were clinically and histopathologically diagnosed.In the study,patients with renal failure or liver failure,hypothyroidism,cardiovascular events have occurred and taking lipid-lowering drugs within 3 months should be ruled out.Patients were performed for a detailed physical examination and the PASI score was calculated according to a fixed equation,and the severity of psoriasis was assessed using the PASI score.Collecting the early morning fasting blood samples were tested by the military test center of Nanjing General Hospital.The 10-year Framingham risk score includes age,sex,total cholesterol,HDL-C,systolic blood pressure,and smoking.The Framingham risk score of the two groups is calculated by a fixed equation.Finally,two groups of patients were divided into three groups according to Framingham risk score:low risk(<10%),moderate risk(10-20%),high risk(>20%).The study found that the risk of cardiovascular disease of psoriatic patients were significantly increased compared with those of the control.The risk of cardiovascular disease in patients with psoriasis vulgaris,especially the older(>50 years),the male,was significantly higher.(P<0.05).There was no significant difference in the risk of cardiovascular disease between the mild and the severe psoriasis group(P>0.05).Apolipoprotein A levels were significantly decreased in patient group when compared to controls(P<0.05).Blood pressure,TC,TG,HDL,LDL,apolipoprotein A,apolipoprotein B,apolipoprotein E,lipoprotein a,C-reactive protein,free fatty acid levels of two groups were different,the difference was not statistically significant(P>0.05).Because of the young age of our populations,the majority of patients had a low cardiovascular risk(81.11%%and 89.78%for the patients group and controls,respectively;P>0.05).However,a higher proportion of patients with psoriasis compared with controls had intermediate and high cardiovascular risk levels(16.67%and 2.22%for the patient group,9.49%and 0.73%for controls,P>0.05)Cardiovascular risk of psoriatic patients were significantly increased compared with those of the controls.The cardiovascular risk in patients with psoriasis vulgaris,especially in the older(>50 years),the male,was significantly higher.(P<0.05).This could be due to an increased prevalence of traditional cardiovascular risk factors in people with psoriasis,the effect of chronic inflammation in patients with psoriasis,or a combination of both factors.Patients with psoriasis should be risk-assessed for cardiovascular diseases,and comorbidities should be actively managed.
