Background: Periodontal diseases are among the most prevalent oral pathologies.It is a destructive inflammatory disease of the tooth-supporting tissues caused mainly by microorganisms.The biofilm is considered the primary agent in the aetiology of periodontitis.Thorough disruption and removal of the supra-and subgingival biofilm and calculus are the primary objectives of cause-related initial periodontal therapy.Initial periodontal therapy is the most common and best effective treatment for periodontitis at present.Nevertheless,in the last years,different therapeutic strategies have been proposed to improve the results of initial periodontal therapy,reduce the clinical limitations and shortcomings of standard nonsurgical treatment in the management of periodontitis and hence to avoid the need of periodontal surgical interventions in some patients with advanced periodontitis.They are based on modifcations of standard therapies(such as enhancement of instrument tip designs),on development of new technologies(such as lasers),or development of alternative treatment protocols(eg,full-mouth disinfection).The purpose of this review is,therefore,to update the scientifc evidence about these advanced nonsurgical therapies that have been published in recent years.Method: An extensive narrative literature review that included overview,systematic reviews and randomized clinical trials,etc.Introduced the present situation of traditional nonsurgical periodontal therapy.Three distinct aspects were described and evaluated independently: the modifcation of conventional instruments,the advent of new technologies,and the development of new treatment protocols.Results: Traditional nonsurgical periodontal therapies presents several limitations.Drug therapy can be used as a supplement to the nonsurgiacal treatment.Modification of conventional instruments and standard therapies,could achieve optimal clinical outcomes and make patients get better treatment experience.Adjunctive new techniques(periodontal endoscope,laser,and photodynamic therapy,etc)are more effective.The spray of glycine,have been proposed as an alternative to remove calculus and bioflm during periodontal maintenance phase.Conclusion: Nonsurgical periodontal therapy is an effcacious mode of therapy for patients with periodontitis,irrespective of the instrument used or the treatment protocol performed.Despite the development of new technology,it still appears that nonsurgical periodontal therapy remains the gold standard for the treatment of inflammatory periodontitis.
