Objective: In recent years,along with the research of inter-arm blood pressure difference(IAD),its correlation with vascular disease is accepted by more and more scholars.Through the verification of medical pathology and multiple clinical studies,remarkable differences of inter-arm blood pressure have significance in peripheral vascular disease,cardiovascular disease,stroke and prediction of cardiovascular and cerebrovascular diseases,prognosis and death.The white matter hyperintensities(WMH)is one of the most important performance of a variety of imaging of cerebral small vessel disease(CSVD),and can be indicators of the exist and severity of CSVD.The present study also increasingly prove cerebral small vessel disease and macroangiopathy have continuous unity in the pathological development.The objective of this clinical trials is to find if there is relationship bethween IAD with CSVD,and if IAD can be used as a convenient indicator to determine the presence and severity of CSVD,through measuring IAD and evaluating the degree of WMH.Methods: This is a retrospective analysis of the hospitalized patients from Second Affiliated Hospital of Nanchang University Department of Neurology from Augest to December in 2016,infectious,toxic,traumatic and other encephalopathy patients were excluded,a total of 318 cases.The blood pressure of patients’ both arms is measured 2 times and averaged each arm’ blood pressure to get the IAD.Collect the patients’ information and medical history: age,gender,the history of hypertension and diabetes,smoking and drinking history;records of blood lipid in patients after admission: cholesterol,triglycerides,LDL and homocysteine.Assess the degree of patients’ leukoaraiosis by MRI(0-3).According to the WMH score the patients were divided into 4 groups,and the above collection and measurement information is to be compared and analyzed,find out the difference between the group factors and correlation analysis.Results: There are 318 hospitalized patients,male 162 cases(50.94%),female 156 cases(49.06%),age is 15 ~ 92 years old,average age(67.36-12.29 years).The degree of WMH is 0 points 87 cases(27.36%);1 points 155 cases(48.74%);2 points 59 cases(18.55%);3 points 17 cases(5.35%).Four groups of average age:(58.83±12.42)years,(68.96±10.13)years,(73.54±11.19)years,(75.00±9.89)years,There are statistical differences in the four groups of age(P < 0.05).To analyze the relationship of WMH and age: r value is 0.462,P value is 0.000,which means there is a certain correlation.Average systolic pressure IAD(s IAD)of four groups:(3.66±2.36)mmHg,(4.48±3.60)mmHg,(4.58±3.66)mmHg,(8.59±4.41)mmHg.There are statistical differences in the four groups of s IAD(P < 0.05).Average diastolic pressure IAD(dIAD)of four groups:(4.06±3.66)mmHg,(4.06±3.42)mmHg,(5.01±4.03)mmHg,(6.88±3.52)mm Hg,There are statistical differences in the four groups of dIAD(P < 0.05).To analyze the relationship of WMH and sIAD: r value is 0.164,P value is 0.003,,which means there is a certain correlation.To analyze the relationship of WMH and d IAD: r value is 0.150,P value is 0.007,,which means there is a certain correlation.The patients with WMH are in the group I,comparing with the patients of group 0 without WMH.The compare of s IAD between the two groups of P value is 0.009(P < 0.05),there are statistical differences in the two groups of sIAD;The compare of dIAD between the two groups of P value is 0.325(P(29)0.05),there aren’t statistical differences in the two groups of dIAD.Couclusion:1.There may be a correlation between the IAD and CSVD,which may be used as a predictor of the presence and severity of CSVD,and more samples experiments are needed.2.SIAD may have more monitoring value in the diagnosis of CSVD than dIAD.
