The Clinical Research Progress Of Liver Cirrhosis Prophylactic Anticoagulation Therapy

Background:Liver cirrhosis(LC)is a common and growing public global disease and public health problem.The annual incidence of liver cirrhosis is 17/10 milion.There is a report that global liver cirrhosis deaths increased from around676,000 in 1980 to over 1 million in 2010(about 2% of the global total).However the disease still lack of special treament,the evidence-based clicinal practice guidelines of liver cirrhosis of Japanese gasteoenterology association in 2015 mentioned that now the treatment mainly includes four aspects:nutrition support therpy,antiviral therapy,anti-fibrosis treatment and complications therapy,and there is useless of any anti-fibrosis therapy,which become to very difficult of the treatment in cirrhosis.Coagulation disorders are common in patients with cirrhosis.The traditional view is that liver cirrhosis is in a state of hypocoagulability,there is a risk of spontaneous bleeding,but in recent years more and more researches indicate that the body can show hypercoagulability in patients with cirrhosis,which is an important factor to promote the development of hepatic fibrosis,a important role in portal vein thrombosis.Therefore,it is possible to prevent PVT,anti-fibrosis and delay the progression of cirrhosis through prophylactic anticogulation.Objective:Through the literature review of coagulation system and prophylactic anticogulation of cirrhosis in recent years,so as to provide new direction for the anti-fibrosis therapy of cirrhosis.Methods:In“liver cirrhosis ? liver fibrosis ? hypercoagulability ? portal vein thrombosis?prophylactic anticoagulation”and other key word search to find out the related research about the literature of coagulation system and prophylactic anticogulation of cirrhosis included in the Chinese knowledge network database-CNKI,Wanfang digital journals group-Chinese biomedical literature database-CBM database and Pubmed et al,to get a comprehensive evaluation.Result and conclusion: Through analysis of a large number of literature,we found that cirrhosis can achieve hypercoagulability by a variety of compensatory;Thiskind of hypercoagulability not only involved in the development of liver fibrosis,and promote the formation of the PVT.Many clinical studies have found that prophylactic anticogulation therapy may prevent PVT and anti-fibrosis in a way,so as to improve the prognosis of patients with cirrhosis,improve the overall survival.Therefore,prophylactic anticogulation therapy is expected to become a new diction of antifibrosis therapy of liver cirrhosis.
