The Value Of Serum CA125,HE4 And Blood Lipid In Endometrial Carcinoma

Objective: Comparative analysis of serum cancer antigen 125,human epididymis protein4,and blood lipid in different endometrial cancer patients with pathological characteristics,different stage and lymph node metastasis difference between,and the analysis of the CA125,the differences between HE4 in endometrial cancer patients and healthy women.To explore its application value in the marker of endometrial carcinoma the effective method for early diagnosis of endometrial carcinoma and to evaluate the feasibility and effectiveness of preoperative diagnosis of endometrial carcinoma with or without lymph node metastasis.Methods: Statistics from July 2013 to June 2016 in our hospital for treatment of gynecological tumor after preoperative diagnostic curettage or postoperative pathology confirmed 263 cases of endometrial cancer patients.Exclusion criteria: 1.Received preoperative radiotherapy or neoadjuvant chemotherapy patients before.2.Taking lipid-lowering drugs.3.Had a history of other malignant tumor and blood system diseases such as breast cancer,thyroid cancer,patients with megaloblastic anemia.After exclusion,a total of 212 patients were enrolled in this study.Patients with the oldest 81 years old,the youngest 32 years old,the average age of 53 years.According to the 2009 FIGO staging of endometrial cancer: phase I: 174 cases(82.1%);phase I above 38 cases(17.9%)(of which: stage II: 12 cases;stage III: 23 cases;stage IV: 3 cases).The pathological types: type I(endometrial adenocarcinoma):183 cases,type II(special pathological type):29 cases.The study included patients admitted to the hospital after conventional detection of CA125 and blood lipid part.With the simultaneous detection of HE4.Treatment is a comprehensive treatment based on surgery,the surgery: 108 patients underwent extensive hysterectomy(50.9%),76 cases underwent extensive hysterectomy and retroperitoneal lymph node dissection(35.8%),10 cases of extensive hysterectomy double accessory resection of retroperitoneal lymph nodes cleaning(4.7%),18 cases underwent hysterectomy surgery(8.5%).A total of 86 cases of retroperitoneal lymph node dissection,71 cases of lymph node negative,15 cases of lymph node positive.The positive rate was 17.4%.Results: 1.Advanced endometrial cancer patients with CA125 were significantly higher than those in healthy women group,P<0.05,and the later stage,the higher the CA125 level,and early(stage I)between the level of CA125 in endometrial carcinoma patients and healthy women group,the difference was not statistically significant.At the same time,between the deep myometrial invasion of different and different the pathological type,CA125 level have significant difference,P<0.05,and in the lymph node between negative and positive group P>0.05,there was no significant difference.2.Endometrial cancer patients with HE4 were significantly higher than those in healthy women group,the later stage,the higher the level of HE4,P<0.05,and early(stage I)there was a significant difference between the HE4.The level of endometrial cancer patients and healthy women group P<0.05,at the same time,compared with no different depth of invasion and lymph node metastasis in the muscle layer,the level of HE4 had significant difference in P<0.05.Between different pathological types,the difference was not statistically significant,P>0.05.3 There was no significant difference in blood lipid analysis in patients with different stages of endometrial cancer(P>0.05),but there was significant difference between lymph node metastasis and lymph node metastasis(P<0.05).Conclusion: 1.The depth of infiltration of CA125 in patients with endometrial carcinoma muscle layer,pathological type and differential stage prediction,has a certain significance,but in the early diagnosis of endometrial carcinoma and predicting lymph node metastasis value.2.HE4 not only in patients with endometrial carcinoma muscle layer infiltration depth and staging,and has a certain significance.The value in the early diagnosis of endometrial carcinoma and predicting lymph node metastasis is higher than CA125,the difference between different pathological types is not obvious.3.Total cholesterol、triglyceride、high density lipoprotein、low density lipoprotein in predicting lymph node metastasis in patients with endometrial cancer may have a certain reference value,the mechanism of formation and significance of the difference is not clear.A large sample study needs to be conducted.
