Clinicopathological Characteristics And Prognosis Associated With HER-2 Expression And Lauren Classification In Gastric Carcinoma

Objective: To analyze the clinicopathological characteristics and prognosis associated with HER-2 in gastric carcinoma by detecting the expression of HER-2,so that we can come up with individual therapise for different patients.Methods: The clinical date of 276 patients,who was proved to be with gastric carcinoma,was complete clinicopathological and follow-up date from March 2012 to December 2013 in our hospital was analyzed retrospectively.To research the clinicopathological characteristics and prognosis associated with HER-2 in gastric carcinoma including different gastric carcinoma of Lauren classification through the way of IHC and FISH detecting techniques.Results: Tumor size,degree of tumor invasion,lymph node metastasis and tumor stage of gastric carcinoma patients were various in different HER-2expression(P<0.05).Gastric carcinoma patients with HER-2 positive expression were more likely than the oppsite to have a bigger tumor size incidence of great than or equal to 4cm(67.4%/50%,P=0.031);Its invasion depth was less confined to the serosa(19.6%/56.5%);Infiltration outer serosa were more in patients with HER-2positive expression than oppsite(80.4%/43.5%);Moreover,patients with HER-2 positive expression had higher incidence of lymphatic metastasis(15.2%/43.9%)as well as higher ratio of TNM stage IV patients(19.6%/2.2%).In gastric carcinoma patients,HER-2 positive expression was more likely to occur in intestinal gastriccancer(80.4%/52.2%,P=0.002).The positive incidence of HER-2 was 16.7%(46/276)in276 patients of gastric carcinoma.Including 23.6%(37/157)in patients of intestinal gastric carcinoma,7.1%(6/84)in diffuse gastric caner and 8.6%(3/35)in mixed gastric caner.There was significant difference between three groups(P < 0.05).Univariate survival analysis revealed that prognostic factors for gastric carcinoma patients were tumor size,tumor site,T stage,N stage,TNM stage,Lauren classification and HER-2expression.Yet,multivariate analysis revealed that only tumor location,T stage,N stage,TNM stage and Lauren classification were independent prognostic factors for gastric carcinoma patients.There was significant difference in 3-year survival rate(28.3%/48.7%,P=0.01)between gastric carcinoma patients of HER-2 positive expression and HER-2 negative expression.Conclusion: HER-2 expression is closely related with Clinicopathlogical characteristics and prognosis of Lauren classification in gastric carcinoma.The prognosis of patients with gastric cancer is better for HER-2-positive expression than negative expression.
