ObjectiveTo realize the current situation of caregiver burden,anxiety and depression in the spouses of patients with breast cancer,and they received the mindfulness-based stress reduction(MBSR)for 8 weeks,and evaluated the effectiveness of MBSR on caregiver burden,anxiety and depression in spouses of breast cancer patients.At the same time,exploring the mediating effect of caregiver burden,and revealed the mechanism of MBSR further.Methods100 cases of breast cancer patients' spouses were as research objects.According to the order of admission,the spouses were divided into experimental group and control group,and 50 cases in each group.91 of them completed the study finally(46 in control group and 45 in experimental group).Participants in two groups received routine care.In addition,participants in experimental group received MBSR once a week for 8 weeks.Before and after the intervention,spouses of the two groups were asked to complete self-designed general information questionnaire,Zarit Caregiver Burden Interview(ZBI),Hamilton Anxiety Scale(HAMA)and Beck Depression Inventory(BDI)to collect breast cancer patients' pathological data,their spouses' demographic data,and detected the effectiveness of MBSR on caregiver burden,anxiety and depression in spouses.The data was input by SPSS 21.0.?2 test,Fisher exact test and t test were used for statistical analysis.Regression analysis was used to discuss the mediating role of caregiver burden.ResultsThe scores of caregiver burden,anxiety and depression were statistical significance between pretest and posttest in the experimental group[(25.98±6.00)vs(22.31±5.76),(17.93±4.65)vs(14.98±4.75),(11.87±3.51)vs(8.47±3.21),P<0.001].While the scores of caregiver burden,anxiety and depression were not significantly improved in the control group(P>0.05).The differences of scores of ZBI,HAMA and BDI in the experimental group were significantly higher than those in the control group(t=-11.21,P<0.001;t=-7.99,P<0.001;t=-7.29,P<0.001).MBSR predicted the difference of score of depression(?=-0.615,P<0.001,R2=0.378).MBSR predicted the difference of score of caregiver burden(?=-0.763,P<0.001,R2=0.582).Caregiver burden predicted the difference of score of depression(?=-0.452,P<0.001,R2=0.464).So the caregiver burden played a mediating effect between MBSR and depression.Conclusions1.MBSR can effectively help the spouses of breast cancer patients to reduce caregiver burden,relieve anxiety and depression and improve their mental health.2.The caregiver burden played a mediating effect between MBSR and depression.But caregiver burden had no mediating effect between MBSR and anxiety.3.The medical staff should pay attention to the spouses of patients with breast cancer,and provide them some psychological interventions to improve their mental health.
