Objective:Chronic atrophic gastritis is a well-recognized major precancerous disease of gastric cancer,serum pepsinogen and gastrin-17 were established as the biomarkers of atrophic corpus and antral gastritis,respectively.Here we analyze the change of serum levels of PG and G-17 according to the various patterns of chronic atrophic gastritis in Guangxi Region,to objectively evaluate the clinical significance of serological test as a screening method for chronic atrophic gastritis.Methods:125 cases were collected who underwent endoscopic examinations in the people's Hospital of Guangxi Zhuang Autonomous Region from December 2015 to August 2016.They were divided into two groups of control(including normal and non atrophic gastritis)and atrophic gastritis group according to the pathological findings,and the atrophic gastritis group were further subdivided into group A(C1),group B(C2+C3),group C(O1+O2+O3)and group D(O4)based on the Kimura-Takemoto classification of chronic atrophic gastritis.The serum levels of PGI,PGII,PGI / PGII(PGR)and G-17 were measured,and the changes of above serum levels among different groups were analyzed.Results:1.Control group consisted of 56 patients and atrophic gastritis group consisted of 69 patients(group A,B,C,D consisted of 21,31,12,5 patients,respectively).2.The mean serum concentrations of PGI and PGR were significantly decreased in atrophic gastritis patients compared with control group(P<0.05),and that atrophic antral with corpus gastritis(group B+C+D)showed lower PGI and PGR levels than those of atrophic antral gastritis(group A)and control group(P < 0.05).In the atrophic gastritis group,the mean serum concentrations of PGI and PGR showed a gradual and steady decrease as endoscopic gastric mucosal atrophy progressed and the differences of PGR levels reached a statistically significant.The mean serum concentrations of PGII was significantly higher in atrophic gastritis patients compared with control group(P<0.05),but showed no difference between the groups in the atrophic gastritis patients.3.The mean serum concentration of G-17 were significantly decreased in atrophic gastritis patients compared with control group(P < 0.05),and showed no difference between the groups in the atrophic gastritis patients.Conclusions:1.Both of the serum levels of PGI and PGR(especially PGR)show a gradual and steady decrease as endoscopic gastric mucosal atrophy progressed.2.Combined detection the serum levels of PG and G-17 has certain reference value for predicting the location and extent of gastric mucosal atrophy.3.It may have a certain clinical value in monitoring the development of gastric mucosal atrophy by detecting the serum levels of PGI and PGR dynamically.
