Analysis Of Conservative Mastectomy For Early Breast Cancer

Objective To analyze the feasibility and safety of Conservative Mastectomy and immediate breast reconstruction in the treatment of early breast cancer,and to reconstruct the cosmetic effect of breast.Methods The postoperative complication rate of patients with early breast cancer treated by conservative mastectomy(CM)and immediate breast reconstruction from December 2012 to February2017 at the First Affiliated Hospital of Guangxi Medical University was analyzed.Recurrence and distant metastasis rate,postoperative3-year survival rate,compared with the same period of breast cancer modified radical surgery(MRM)patients to do paired analysis.The retention of breasts was better than that of breast mastectomy(NSM)and breast resection(SSM).Results A total of 55 cases of CM surgery and 17 cases of SSM were performed.Among them,5 cases of breast reconstruction were performed and 12 cases were autologous reconstruction of pedicled latissimus dorsi myocutaneous flap.Forty-three patients underwent 40 patients with NSM and 23 patients underwent prosthetic breast reconstruction.Two of them underwent prophylactic resection and reconstruction of contralateral breasts.Fifteen patients underwent reconstruction of the dorsal latissimus dorsi myocutaneous flap.There was no significant difference in the postoperative complication rate between CM and MRM(10.9% vs 12.7%).P = 0.768 by chi-square test.The three-year survival rate was 100%.There was no significant difference in recurrence,metastasis rate and survival rate within 3years after operation.All of them were analyzed by chi-square test(P>0.05).CM patients were satisfied with the effect of reconstructing breast beauty in 48 cases,dissatisfied with 7 cases,the overall satisfaction rate was 87.3%.NSM surgery group,the satisfaction rate was 94.7%(36 cases were satisfactory,not satisfied with 2 cases).There were significant differences between the two groups in the postoperative satisfaction rate of 70.6%(satisfactory 12 cases,dissatisfied with 5 cases)in the SSM operation group,and the chi-square test was P = 0.024.Conclusion: 1,for early breast cancer,especially the low risk of recurrence and prognosis of patients can choose CM surgery,the prognosis is safe;2,CM surgery and MRM surgery group did not increase the rate of postoperative complications of breast cancer patients,Local recurrence rate,distant metastasis rate,CM can retain breast shape after surgery,patients with high satisfaction,especially for small and mild to moderate systolic breast of female patients;3,NSM group and SSM surgery group,NSM group of patients with higher satisfaction with the reconstruction of the breast,the cosmetic effect is better;4,CM surgery with the LDMF immediate breast reconstruction success rate is high,postoperative complications rate is low,the postoperative complications rate is low,But latissimus dorsi myocutaneous tissue is limited,not suitable for large,moderate and severe sagging breast reconstruction.
