Objective:The purpose of this study was to investigate the expression of keratin 14 in discoid lupus erythematosus and lichen planus and its clinical significance.Methods:A total of 20 cases of discoid lupus erythematosus embedded in paraffin-embedded skin of the First Affiliated Hospital of Guangxi Medical University were selected and 20 cases of lichen planus.The expression of keratin 14 in lichen planus,discoid lupus erythematosus and normal human epidermis was analyzed by SP immunohistochemical method in 10 cases of normal skin tissue after orthopedic surgery.The results were measured by semi-quantitative method of artificial evaluation,and the average optical density was measured by Image-Pro Plus 6.0 digital medical image analysis system,The results of the two methods using SPSS 17.0 software for rank sum test and t test and other statistical methods,P<0.05 was statistically significant.Result(1)Keratin 14 is expressed in the whole layer:10 cases of normal skin group in the basal layer or the lower part of the spine were positive expression,20 cases of DLE group and 20 cases of LP group were expressed in whole layer,However,the positive expression of DLE group was mainly concentrated in the middle of the spinach,LP group was more uniform,Three groups were compared between two groups,All P<0.05,the difference was statistically significant.(2)Expression of keratin 14 in the basal layer:20 cases of DLE group,2 cases were weakly positive,the remaining 18 cases were positive expression,LP group,normal skin tissue were positive expression,All P<0.05,There was no significant difference between the two groups.(3)Keratin 14 is expressed in the lower part of the spinous layer:20 cases of DLE group,20 cases of LP group were positive or strong positive expression,And only 10 cases of normal skin tissue were positive expression,The positive expression was mainly located in the cytoplasm of keratinocytes,Compared with the control group,P<0.05,the difference was statistically significant,And the two experimental groups were compared with each other,P>0.05,the difference was not statistically significant.(4)Keratin 14 is expressed in the middle of the spinous layer:two patients in the DLE group had no expression,and the remaining 18 patients were positive or strongly positive,LP group was strongly positive expression,normal skin group was not expressed,Positive expression was mainly located in the cytoplasm of keratinocytes,3 groups were compared,all P<0.05,the difference was statistically significant.(5)Keratin 14 is expressed in the upper part of the spinach:Six patients in the DLE group had no expression,and the remaining 14 were strongly positive or strongly positive.LP group was strongly positive or strongly positive expression,normal skin group was not expressed,Positive expression was mainly located in the cytoplasm of keratinocytes,3 groups were compared,all P<0.05,the difference was statistically significant.Conclusion(1)Keratin 14 expression in the LP and DLE groups increased,suggesting that keratin 14 may be involved in a certain extent,the occurrence and development of both.(2)The expression of keratin 14 in the middle of the acupoints and the upper part of the acanthium in the LP and DLE groups were different,Suggesting that there may be differences in the regulation of proliferation and differentiation of both keratinocytes,The exact mechanism needs further study.(3)The expression of keratin 14 in the LP group was stronger than that in the DLE group,Especially in the upper part of the spinous layer was relatively strong positive expression,May be a useful reference for both differential diagnosis.
