Purpose: To study the value of dynamic fixation device Coflex in the prevention of adjacent segment degeneration after fusion.Methods: From January 2015 to January 2017 were retrospectively analyzed in clinical data of 30 cases treated in our hospital with PLIF and Topping-off patients.14 cases treated by Topping-off,of which 7 were male,7 were female,age range 46-71 years,the average(58.00±9.33)years;should be treated with PLIF surgery in 16 cases.There were 8 males,8 females,age range 47-77 years,mean age(60.25 + 7.83)years old.The change of lumbar lordosis and placement of Coflex section To evaluate the role of Coflex device in the prevention of fusion adjacent segment degeneration(PLIF),which is the Pfirrmann index of the intervertebral height index,the intervertebral activity,and the changes of the intervertebral disc.Results: All patients completed surgery,surgical procedures were not found in the nerve roots and cauda equina injury;have no perioperative infection,wound healing is good;were followed up for 8-24 months,average(14 + 5)months,there were 1 cases in group.Topping-off at the spinous process fracture and extension the bed days,natural fracture healing.During the follow-up period,all patients had no internal fixation of displaced,shedding of.PLIF group at the end of the follow-up intervertebral height index decreased compared with preoperative,lumbar activity significantly increased compared with pre operation,the group difference was statistically significant(P < 0.05).Topping-off group Coflex into the segmental intervertebral disc height index obviously increased compared with preoperative,although the later has lost,but at the end of the follow-up is still slightly increased compared with the preoperative,the group difference was statistically significant(P < 0.05);at the end of the follow-up placement of Coflex intervertebral activity compared with preoperative decreased slightly the difference was statistically significant(P < 0.05).At the end of the follow-up,two patients with lumbar lordosis compared with preoperative had a little change,but the differences were not statistically significant(P > 0.05).At the end of the follow-up group PLIF fusion of adjacent segment 2 cases Pfirrma The NN increase for 1 grade,for the imaging of ASD.Topping-off group Coflex treated segments no intervertebral disc Pfirrmann grade increased.Conclusion:Interbody fusion can cause the activity of adjacent segments increased to adjacent intervertebral space height loss,accelerate the degeneration of adjacent segments.In the fusion of adjacent segment into the Coflex device can be restored into the height of the intervertebral space to a certain extent,effectively avoid the resulting from the fusion activity of adjacent segments increased and intervertebral height loss,which has a positive effect on the adjacent segment degeneration after fusion of prevention.
