The research background and purpose: Long bone defect of tibia has always been a big problem in clinic and the bone transport technology open the prospects for this problem,but with the use of this technology,more complications,long treatment process such as problems also emerge.How to reduce the occurrence of postoperative complications and promote transporting area of bone growth and mineralization has been attracting the attention of scholars both at home and abroad.This topic by observing bone transport combined with traditional Chinese medicine(TCM)postoperative Taohong Siwu decoction intervention treatment of long bone defect in tibia to study the method of analysis,further observe bone transporting technology for the treatment of long bone defect of tibia bone effect,and explore the influence of traditional Chinese medicine Taohong Siwu decoction on bone transport process,aims to find a feasible method of Chinese and western medicine can reduce bone transporting the postoperative complications,promote the healing of bone bone in the process of transporting,improve the quality of treatment.Methods: Retrospective study the first affiliated hospital of Shandong University of TCM orthopedics in February 2014 to October 2016 were 28 cases of treatment of long bone defect in tibia bone handling technology application cases within the scope of defect in 6~15 cm,and intend to choose to take Chinese traditional medicine is divided into two groups according to the patients.Among them,14 cases of taking,as Chinese traditional medicine group,14 patients not taking over,as the control group,traditional Chinese medicine(TCM)for Taohong Siwu decoction.28 cases according to the preoperative X-ray and CT measurement of bone defect in 6~15 cm.All patients admitted to hospital after debridement and anti-infection treatment actively,according to the circumstance to inflammation such as stable condition after all use Ilizarov circular external fixation frame line handling operation of bone cutting.After surgery,the control group given anti-infection,anticoagulant,vasospasm treatment,routine wound dressing,X-ray,periodic review to guide function exercise;Based on Chinese traditional medicine group in the control group take a Taohong Siwu decoction,1 times a day,six weeks in a row.Observe and record in the process of transporting treatment bone pull pain major complications,such as measuring calculation bone 4,8,12 weeks after transporting began to pull area callus diameter ratios(CDR).After the completion of the pull followed up for 3 months.When X ray film are open area is dense fog shape cutting bone callus,CDR value>85% or X ray film of judgment and side four cortex position at least three mineralized completely except to a fixed frame.Finally,collection of two sets of major complications,4,8,12 weeks in the process of handling the CDR value,transporting after remove the data such as the time needed for external fixator.Finally,the collection of data,data statistics and comparative analysis.Results: All patients received 6~24 months follow-up.All patients were eventually gain clinical healing and dismantling external fixator,19 cases of bone segments within three months after the join forces with direct healing.Bone handling time is required for the external fixator dismantling,traditional Chinese medicine(TCM)elder sister are the average number of days with the control(193.14±44.54),(227.71±39.29)days,the difference is statistically significant(p=0.038,p<0.05).Measurements of 4,8,12 weeks of Chinese traditional medicine group pull area callus diameter ratios(CDR)were higher than the average of the control group,with statistical difference(P<0.05).Complications: 28 cases of 15 cases with pull pain(4 cases of traditional Chinese medicine group and control group in 11 cases),to stop moving after 2 days of symptoms,continue to move not hemp pain symptoms;10 cases of nail infection(Chinese traditional medicine group in 2 cases,control group 8 cases),postoperative local treatment under control;Meet client don't heal 9 cases(4 cases of traditional Chinese medicine group and control group 5 cases)5 cases after end pressure healing is good,open extremities shaping pressure and fixed in 4 patients after healing.According to the statistical analysis,Chinese traditional medicine group pull pain,infection of needle,the occurrence of complication rate is significantly lower than the control group,there are significant differences(P<0.05),two groups join forces with side don't heal showed no significant difference(P>0.05).Conclusion: From the perspective in the study,bone transporting technology for the treatment of long bone defect in tibia bone effect is satisfactory.Taohong Siwu decoction on bone segments can promote pull move to complete the process in the process of bone growth,shorten the treatment time,and to a certain extent,can reduce transporting pull pain,bone needles,the occurrence of complications such as infection,relieve patients' pain,improve the quality of treatment.
