Background and objective: With the development of modern image display technology, 3D technology has come into our life. While 3D laparoscope system has been widely accepted by surgeons because of its better visual effect and safer operations compared to traditional laparoscope system In the medical field, and 3D thoracoscopy in the surgery whether the same advantage has not been confirmed.Through summarizing and analyzing patients clinical data in our hospital who underwent 3D assisted thoracoscopic surgery and 2D conventional thoracoscopic resection of lung cancer, to evaluate the clinical value of 3D thoracoscopic surgery in lung cancer surgery. Methods: From August 2016 to March 2017, consecutive 140 patients with lung cancer were recruited in the department of Thoracic Surgery of Yunnan Tumor Hospital, including 50 patients who received 3D video assisted thoracoscopic surery(3D group) and other 90 patients receiving 2D video assisted thoracoscopic surery(2D group), all patients underwent lobectomy and systematic dissection of mediastinal lymph nodes. The operation time, Operative blood loss, Lymph node number, Length of stay and other perioperative parameters between the two groups were recorded and compared. Results: Two groups of patients were completed surgery successfully, no perioperative death. A signifcantly less operative time was found in the 3D VATS group (110.12 ± 31.92) than in the 2D VATS group (122.66 ± 32.49)(P<0.05); the Operative blood loss in the 3D group (98.8 ± 57.87) than the 2D group (123 ±58.15)also decreased significantly (P < 0.05); and the postoperative chest drainage in group 3D (519 ± 298.71) than the 2D group (546.02 ± 301.26) is also reduced, but there was no statistically significant difference (P > 0.05). No statistical difference was found between the two groups with gender (x2 = 0.37), age (t = 0.72), tumor location (x2 =0.64), p - TNM staging (?2 = 0.89), Pulmonary function FEV1% (t = 0.48), DLCO%(t = 0.41), The conversion rate (?2=0.68), postoperative pathological diagnosis (?2=0.37), postoperative chest tube retention time (t = -0.29), postoperative hospital stay(t=-0.22) (P >0.05).Conclusions:3D VATS is a new technology, which is helpful to promote the improvement of endoscopic therapy. Compared with the 2D system, 3D thoracoscopic surgery is safe and feasible in the treatment of lung cancer, the blood loss is reduced and the operation time is shorter. It is worthy popularizing.
