Clinical Observation Of Tosight Video Laryngoscope Applied In 130 Difficult Intubation

Objective:To observe the clinic effect of Tosight-B-blade video laryngoscopy in different extent difficult intubation.Methods:A retrospective and clinical control study was applied.Data from all difficult intubation patients who underwent selective plastic surgery and required orotracheal intubation using To sigh t-B-blade video laryngoscope from January 2008 to December 2016 at our hospital was prospectively recorded in a paper-based database.The patients were divided into mild group,moderate group and severe group accroding to preoperative evatuation of airway.Anesthesia ways,intubation methods,Cormack—Lehane grade under Tosight video laryngoscope,intubation time,intubation attempts,intubation success rate,whether to replace the operator and complications such as teeth and soft tissue injury,sore throat and vocal cord injury were observed and recorded.RESULTS:130 patients were enrolled in the study.Among which,127 of them was procedured successfully under Tosight video laryngoscope and 3 of them was changed to other airway tools.Besides,two patients was adopted mild sedative intubation which spontaneous respirations were retained.In mild group,moderate group and severe group,Cormack-Lehane grade 1 view achieved by Tosight video laryngoscope were 31(96.9%),60(92.3%)and 27(81.8%)respectively.First-attempt success rate were 90.6%,89.3%and 81.8%respectively.Average intubation time were 52.8±49.5s,67.1 ± 58.3s and 78.1 ± 78.1s respectively.Besides,the rate of replacement the operator for a more experienced one were 12.5%,24.6%and 27.3%respectively.However,the differences were statistically insignificant(P>0.05).Compared with mild and moderate group,more complications were occurred in severe group significantly(P<0.05),especially for those with two or more complications.Conclusion:Effective airway evaluation contributes to ascertain the degree of difficult intubation.Tosight video laryngoscope is able to solve a variety of types and different degrees of difficult intubation quickly and efficiently.However,it is limited for Tosight video laryngoscope to be used in patients with serious limited mouth opening,sereve mandibular hypoplasia,serious head,facial and neck scar or large build.Besides,when using the laryngoscope,60° to 80° endotracheal tube bending Angle applies to most difficult intubation.
